I'd do all I could to get onto another account. I was put on an account like that with all dictations under one minute, usually only 30 seconds, and on top of that, I had to fill in the demographics for each patient on a spread sheet! Quit that MTSO when they had nothing else to offer. If MTSOs knew what they were doing they would double the line for those clients!
