I saw a transcription note by a scribe. - What slop. SM

Absolute slop.  Things not spelled properly,  formatting completely in the toilet.   I know that note was nowhere near even 98% quality. 

I will also bet that scribe was nothing but an admissions clerk,  a CNA at best, recruited to scribe.

My husband was recently in the ER and I got a copy of his note,  a scribe did it, but she was an RN.   Still a  pass-poor job of it.  I'd grade that at 80% quality.

Sad.   Really pathetic.   Then the hospital wonders why I called and chewed their A over the ER report?   The clinic wonders why I called and complained about the errors in his chart and registration information -  even after I corrected the clerks at the time of visit. 

Nothing but a mess.  I will warn everyone to get your records and check them carefully, look at every little detail to make sure it's right.
