not dictating mg with dose...what to do? - southpaw

The doctor is not including the word mg with doses of medication. Do I just transcribe what I hear or insert it?

IE- simvastatin 10, vitamin E 400 b.i.d., metformin 850 b.i.d., ibuprofen 400 p.r.n

mg or not - nm

No don't assume that it's mg. Vitamin E could be IU, international units.

need to know what the correct one is to use.

What does your employer/client say?? They are the - ht

ones you need to ask. Where I work, we add them, but not all are like that, especially if typing verbatim. Ask them.

Supposing you transcribe verbatim:

CURRENT MEDICATIONS
1. Aspirin 81.
2. Coumadin 3.
3. Ambien 10.

etc... Doctors need to be alerted this is a potential threat to patient safety. If this falls into inexperienced hands, it could be mistaken for instruction to give the patient that many actual pills instead of mg, mcg,IU, or whatever is actually intended. I'd flag to QA, or what ever is the kosher process for reporting dangerous mistakes to your superiors, if you are not allowed to verify and correct it yourself.

I'm having a little trouble with this medication dictation.   "3.  Methotrexate 25 mg/mL 0.9 cc weekly injection."  Then she says "4 needles 1 mL (s/l u or unit)/125 g1." What I can't figure out is if she is saying 4 needles or if she is dictating the #4 and "needles" is a medication that I can't find a match to online.  Can anyone shed some light here, please? ...

Pristiq Dose 2 - 25 Mg Daily Or 225 Mg?

Hey all, I am trying to figure out if Pristiq can be dosed at 225 mg?  The website I use for dosing doesn't say that.  The doctor says, "two twenty-five mg daily."  He doesn't say two hundred and twenty-five mg daily.  However, I see that Pristiq comes in 50 mg, so if the dose was 50 mg why would he say 2 - 25 mg?  LOL!  Just checking to see if anyone has an idea. Regarding the DLP - I've seen it written variously mGy-cm, mGy•cm and mGy.cm.   Becuase it's a product, my inclination is think that the second (with the dot used as in mathmetical publications to indicate multiplication) is correct but just not sure.  Anyone have a definitive answer & reference?   Thanks :)  ...

I very seldom vent or complain, but it has just got to me today.  This surgeon does such good dictation, EXCEPT when he yawns and continues to dictate.  How in the world does he think we can understand him when he does this.  It is not a little yawn either.  It almost makes me shutter when he does it.  I am sorry, I just had to finally vent after listening to him for 10 years. ...

He does this all the time...turns a 1.5-2 min dictation into a 15 minute dictation.  I fast forward through it, but I still have to "listen" to the whole file before I can turn in the dictation.  This is on top of being a horrible dictator; he whispers, shuffles papers, etc, and has extremely long pauses between words.  Gah!!!! Okay, just had to get that out of my system.  Thanks! ...

I have always thought the numbers less than 10 should be in numeral form if referring to medication.  So I thought 1 tablet of Vicodin a day would be the appropraite way to type it or 2 tablets of Vicodin a day.  However, I am not sure.  Any ideas? ...

I'm going freaking nuts!  I am trying to transcribe a resident's dictation (bad enough), but she's smacking and chewing gum throughout the whole thing.  It's so loud that I can hardly make out what she's saying.  I don't very often rant and rave, but ... Okay, if you're going to have an RN, FNP, or PA dictate for you, at least make sure they know the terminology!    I just suffered through at 10 minute discharge summary on a patient post CVA, and this lady was reading the CT scan and MRI reports of the brain into the record with clearly no idea at all of what she was reading or what it meant or how to pronounce the parts of the brain.  Thank goodness I've been around long enough to be able to get and idea and fill in so ...
