Nature and duration as a heading?? - jenny

Posted: May 12, 2012

I hear the doctor say Nature and Duration...Patient was admitted to ___ [PLACE]..... Is that right??? need serious help!!

You will need to ask - your employer

[ In Reply To ..]
what the specifics are for that particular account. Every client is different, and we have no way of knowing what that client wants. Your employer will be able to tell you.

Heard it before? - jenny

Have you ever heard of that before though??

It doesn't matter what anyone else - has heard

What matters is what the client wants, and we have no way of knowing that.
Do not think Jenny means Heading heading; sounds more like - a s/l place. n/m

Here's my take on it, Jen. If it sounds like a heading, make it a - heading, sm

It looks like your sentence after NATURE/DURATION: -- NATURE AND DURATION: is (Patient was admitted to whatever the name and blah, blah, blah.) a complete sentence without nature and duration included.

So IMO, I would make anything that is not a part of the sentence a heading, i.e., HEENT: X-RAYS: as long as it reads like it should be a heading. Hope that makes sense and as the other poster said, if you have the account specs or a sample, go by that first.

Nature Of The Beast - MTSOs
Oct 22, 2012

Fellow MTs - Would appreciate any and all feedback. I am in need of gathering information on the trends of the medical transcription profession. If you have worked as a hospital-employed MT as well as for an MTSO (medical transcription service organization, or MT outsourcing company), these are examples of the type of input I am requesting: 1) How would you characterize differences in pay rates, benefits, and potential gross income between hospital vs. MTSO employment? 2) Ha ...

Question Regarding Human Nature And Changes
Jul 10, 2014

The only constant, when working in health care, is that things are going to change. A wise MT told me that years ago, and boy has she been right. I do see a wide variability in how people handle change. I've seen it in MT, and I've seen it in coding, with the transition to ICD10. It seems some people embrace the change, learn from it and become even more successful, while others decide it's too difficult, complain about it and become bitter.  I have several more years until I ...

Heading?
Apr 22, 2010

This doctor describes the preop/postop diagnoses, type of anesthesia, and gives the surgeon's name (all of which I made separate headings for as it seemed he was dictating them). After the surgeon's name, he goes into a short description of the patient and a few physical exam findings as they pertain to the procedure.  He does not dictate a heading or anything, although after his description of the patient, he dictates a new paragraph and a PROCEDURE heading.  Should I put a ...

Perineal Exam--heading In PE
Feb 18, 2010

I have a couple of doctors who dictate without giving headings--I'm supposed to add these.  What heading would I put "perineal exam" under in this report? He dictates it after the abdominal exam, but would that be the appropriate section to put this under?  I thought of pelvic, but it really isn't a pelvic (female patient). ...

What Is Wrong With DQS 7.1's ASR Heading System?
Sep 03, 2012

I thought that DQS 6 was bad, but this 7.1 certainly topped that one.  It seems to just randomly makes words headings whenever it feels like it.  Just now, on a dictation clear as day, in the middle of a sentence, it decided that the word "the" was to be a heading.  Why? ...

Concerning The Discussion A Few Days Ago About Which Heading Gait Testing Goes Under.
Jun 27, 2015

Gait testing is part of the neurological examination and should therefore go under that heading.  From my perspective, healthcare document integrity should hold sway over account specs or doctor preferences.  Too many doctors think all that matters is what they say or how they interpret what is said, which is total bunk.  If that were true, we would all just start typing melanotic stools every time one of these medical geniuses dictates that because certainly there isn't a do ...

Got A Critical Error For "omitting Dictation Of A Critical Nature."
Jan 04, 2010

The dictator said "... will get cardiac enzymes and will get a carotid Doppler in the morning...", or something similar, I have deleted the report.  I left out the second will.  This is not a verbatim account.  Critical error for omitted dictation pertains to lab results, normal values, etc.  I didn't think the word will meets that criteria.  I also had Tylenol No. 3 changed to Tylenol #3.  The drug lists still show it as No. 3.  ...
