Got a critical error for "omitting dictation of a critical nature." - mt

The dictator said "... will get cardiac enzymes and will get a carotid Doppler in the morning...", or something similar, I have deleted the report.  I left out the second will.  This is not a verbatim account.  Critical error for omitted dictation pertains to lab results, normal values, etc.  I didn't think the word will meets that criteria.  I also had Tylenol No. 3 changed to Tylenol #3.  The drug lists still show it as No. 3. 

The Quality Coaches are the ones doing the auditing/grading. - I would ask her.

I have also noticed changes in my reports that don't need to be there. If I send a report to QA with 1 blank in it, and then review it, QA will have made other changes in the report.

Most of the time, they change words that don't need to be changed, or ad words that I omitted for redundancy (like you did). My account is not verbatim either.

The nitpicking is getting out of control. I hope that you are going to complain to the powers that be about this. A critical error? I think not! Not an error at all!

I don't have a problem with valid correction and want to know if I make real errors. Emphasis on real. The rest is nothing but pointless, irritating micromanagement that has absolutely no effect on patient care, just making this job harder than it already is.

Saunders Pharmaceutical Word Book: Tylenol No. 3. The heck with the Internet. Books have errors, but they're far more accurate than the Internet.

As for your word omission--not sure what the rules are. An omission is an omission, but that should have at least been downgraded. Totally silly to take points off for that.

What?? The BOS even says to use mL instead of cc.

Considering I've already sent 3 emails today because of "corrections" made to my reports that were INcorrect, I'm starting to feel like it doesn't matter. I could type jibberish and they would probably let it go through....oh, but they would change the word 'and' to an ampersand! (&)

New MQ standards say can use "cc" if they dictate "cc". Also say not to use "&" and type out "and". I know I for sure wish they would make up their little pea-brains and quit going back-and-forth on their instructions. I think changes from one person in charge to another.

'and' but they corrected a report today and changed H and P to H&P. I was not aware of the cc change. As far as I knew, we were suppoed to change to mL.

I know, I wish they would make up their minds too! It's so frustrating. I feel like an idiot half the time and I've been doing this for 10+ years!

They made up new "standards" a few months ago. I had to have MANY things verified for my BOB as to changes such as pt's names allowed in text - standards say "no", but most of my accounts say it's OK if dictated. SOO CONFUSING. Told to always go by client profile first. I feel stupid too - have been doing this job since 1972, working for MQ for 12 yrs.

with this job. No matter how many times an explanatory note is left for MTs stating to please check the new MQ Guidelines regardomg cc amd mL, the note just never seems to be read by some MTs.

Please see MQ Guidelines: You will find that cc is not on the list of prohibited abbreviations. Never was. MQ uses the short list. Not the long list that included suggestions to adopt at a later date (that's where the mL for cc was located).

Eventally MMEs completely give up on leaving notes and just do the correction without taking the time to leave the comment. What is the point if it is obvious the same comment is being written for the same person over and over and that person never bothers to read it ?

As for the ampersand, check the list of prohibited symbols in the new MQ Guidelines.

time. I have never been left notes. That is why I have sent emails asking for clarification.

The MQ standards that I have state that ampersands are prohibited. And all of my client profiles say to go by BOS II, which says to use mL instead of cc. Go figure.

How am I supposed to know if I don't get left a note?

are more than likely the exception rather than the rule. I have only had maybe 3 or 4 notes in many years and I have appreciated each of them. The last one was approximately 2 years ago, a very picky client with an extensive CP. The QA person (I don't believe we had MEE then) changed all the headings to what the doc dictated instead of the standard. Before I could even read the feedback she left, the hospital called and had a fit because Dr. Whatever's reports were not on the standard. The poor QA was fairly new and didn't read the CP. I felt a bit bad for her.

In any case, from what I understand the new pecking order is CP first, MQ Guidelines second and BOS 2 third. All of my CPs do not allow cc, want everything changed either mL or cubic centimeter.

Please, keep leaving the notes, we do appreciate your efforts. None of us want to make the same mistake over and over again. We all want to produce the best quality report we can.

Also, anyone new to the Q or DQS, if you hover your cursor over the "Quality assurance feedback" section on the right you should be able to read QA/MEE feedback. If there is feedback left for ADT it will be at the top of the screen under the ADT & Demographics tab.

Your account is not verbatim so you did the right thing, the second "will" is redundant. Most times they preach "verbatim" but nothing is actually verbatim, or the clients would really have a fit. I can see it now, actual verbatim "I, um, the patient and is also, um with, you know, the CKD" even if you take out the "um and you know" it still sounds stupid. I make small minor changes so the dictator doesn't sound like an idiot. Sometimes I get busted for them, the same as some of you have, sometimes not. Which brings me to my next beef so to speak. If an MT/ME is technically correct in accordance to the CP, MQ Guidelines and BOS 2, or is technically correct because it was not mentioned in CP or MQ Guidelines but is shown as being correct in BOS 2 in multiple ways, don't make a correction based on someone else's personal preference. We all have a personal preference, if it is technically correct leave it alone.

As far as the Tylenol No. 3, No. 3 is correct, not #3. According to Rxlist.com, Steadman's drug book and Tylenol.com itself, Tylenol No. 3 is correct.

I've got this terror of critical errors.  I shudder every time I check my feedback, if I've left a marker (sometimes I've noticed they'll check the entire report, not just the marker.  Guess it depends on the MME).  What happens if you get a critical error?  Are you back on QA for awhile?  Do you get nasty e-mails?  Do you get canned?  40 lashes? ...

This is a new one to me and laughable honestly. I thought we were to spell phoenetically a doctor's name in the text if we cannot find it or verify it.  So this is what I did as usual and got a MAJOR CRITICAL ERROR for putting the wrong doc name??? WTH. Seems like they are grasping at straws now. What's the deal? We are to be mind readers now and get these names exact every time even when they don't spell, we can't find, etc. etc. Nuance just makes itself look ridiculous ...

So 3 points off...short report....and, course failed, 88%......I'm doomed for the next 2 weeks.  Probably now way to get up over 99% at this point.   Anyway, so I went back to check on it (I usually don't bother), I was going to try to relisten (still can't for whatever reason)...but what I DID see was that the report was sent to the client with the apparently incorrect value of 0.5 still on the report.   So they find the error but don't correct it?   ...

barely scraping by and passing according to MQ?  I just received an audit that had a score of 99.3%.   I'm told I passed the audit but got a critical error (left out a word) so she will need to do another audit next week.  Fine. But how can I get a critical error (in their eyes anyway) and still "pass" the audit?  More importantly, does anyone know if my pay is going to be docked because of this critical error, or do you just have to get 99% period.   TIA ...

Has anyone at the Q been getting hit by "critical errors" on crazy stuff that has never been an issue BEFORE?   All of the sudden, after no feedback for about a month, no audits, nada, zilch, I got my audit and was given critical feedback for not capitilizing a department! (recovery room, radiology suite, cardiac cath lab...)  AND, in a verbatim report, with a client profile that says specifically, DO NOT ADD A HEADING IF ONE IS NOT DICTATED, for NOT adding a heading!  This makes me nuts ...

GD SOB! Have been passing all the audits with flying colors (99% or better), even the audit earlier this week.  but then I got an email today of a report with critical errors.  Real bad doctor who mumbles and changes his mind in midsentence but doesn't say 'scratch that' or 'correction' or anything like that.  sheesh.  so do they automatically put you on disciplinary action or cut your pay for one bad report?  or do i hope that on my next ...

So I check MQ Central as usual before starting my shift this morning.  Found 4 messages stating critical backlog with weekend incentive.  I sign on.  No jobs.  I'm tier 3.  Never saw NO JOBS first thing on my regular Sunday shifts.  WHAT THE ____?  I read on to get a slap on the wrist from my RTL.  Why didn't I send a report to QA that had multiple blanks.  I tried to be objective, but it was for the usual reasons I send multiple-blank repor ...
