HELP! What type of report is this? - seadog75

What type of report is this? Doctor doesn't dictate cleary what it is.

READ BELOW:

(Type of report? MRI SPINE??)

CLINICAL BRIEF: Low back pain, status post surgery ___. The lumbar vertebrae are normal in number and configuration. No recent bony injury is identified. Hypertrophic degenerative changes are noted throughout the entire lumbar spine. There is minimal disc space narrowing at L3-L4 and moderate disc space narrowing at L4-L5 and L5-S1. There is calcification in the wall of the distal abdominal aorta which appears to be minimally dilated.

IMPRESSION: Mild degenerative disc disease at L3-L4 and moderate degenerative disc disease at L4-L5 and L5-S1."

 

I would guess a back x-ray--not detailed enough for - MRI. NM

It is a lumbosacral spine x-ray. The doc won't need to figure it out. If you take the time to read through the report the doc is referring to different levels of the lumbar spine. No terminology is used that would indicate it is a MRI or CT. I know is it frustrating but sometimes it helps just to sit back and read what the doc has dictated and all of the sudden it is clear as a bell. JMHO but you don't want to be leaving studies for the doc to figure out what they are. A word you cannot understand/question is one thing, but saying you will just let the doc figure it out...all it is going to take is getting the doc on a bad day, or the grumpy doc and he/she will go screaming to the powers that be. Just take your time and try to relax and read the report and alot of times the answers are right there in the report. Often times I will have a word I am stuck on and just can't understand, when I get to the impression the doc gives the same sentence/verbage again and it is clear the 2nd time, which allows you to go back up and fix that blank you could not understand.

I have always typed my initials with the doctor then me as ABC/def.  That is the way I was taught when I got my MA degree and have always seen it as that until I started working for this office.  They type it ABC:def or ABC;def.  Nothing is consistent and seems like everyone does whatever they feel like.  Also, when typing a note for a PA and CRNP I always put their initials and then the initials of the doctor and then mine such as ABC/DEF/ghi and I have not seen one report t ...
