Should this be typed (5/10 or 5 out of 10) for:

Either way really is correct. Where I work if they - j
[ In Reply To ..]say "5 out of 10" that is what we type, if they say "5 over 10" we type 5/10. I think it is according to the client. We also don't use a hyphen for "2 to 3" unless they actually say hyphen. Everyone out there is different, so you need to check with your client/MTSO.

It is written in the patient's chart as "carbevo." I know it is for Parkinson disease. Carbidopa and levodopa. I googled it and found carb/levo.
