standards/templates - New 2 Q

I've been with MQ for about 2 months now.  I've noticed that many, many of the dictators use what we used to call "standards" or templates.  We just inserted the canned text and made changes where indicated.

Is this allowed?  I know some has their own (crtl S), but can I make my own and use them?

What is autocorrect and how do I use it/or is there instructions somewhere in the online manual?

I use the program already in DQS. Highlight the dictator's exam after you've proofed it, hit Ctrl + E, and in the Replace field type your shortcut, then hit Add. The next time you get his or her exam, type your shortcut and it will produce onto the screen. However, you STILL need to do a full listen against the text, as they do make changes sometimes.

I've been using my own shortcuts and templates forever. Why isn't it allowed? My CCM has never said anything about it not being allowed. It's a shortcut for goodness sake. We are production based employees, shortcuts help us be more productive!
Cntrl E is Expansions and Auto Corrections. I do it all the time, too. - ac

i have heard many times that excessively long expansions are NOT allowed.

to make sure there are no changes, don't see the difference. They would have to check every single report (if possible) to track changes to know if one was doing it or not--UNLESS they are not read through and changes made as needed as stated above.

I have whole reports in Ctrl E and on my desktop. I've done this for years. Never have any of my CCMs said a thing.

with PE, ROS, operatie reports, etc., and never told it is not allowed. As long as you listen and make the changes necessary you can use autocorrect expansions however you want.

Them "not being allowed" is because the expansion program will only allow a certain number of characters before it will max out, not because we aren't allowed to use large paragraph expansions.

As long as we proof them, there isn't an issue.

this. Nowhere in any of the materials given us is it stated we cannot use autocorrect to make expansions or that there are any limitations to using it. If you want to greatly improve your production, use this tool. If it was not allowed, then we would not have the autocorrect feature. They want high producers and this is the best tool to achieve this.

A lot of times I also copy and paste from the repository too! Making adjustments of course!

Anyone else have any shortcut hints and tips? I like the one about having some on the desktop. Why not?!

Copy/pasting from DQM into DQs is a definite no-no. If you're caught, it can be big trouble.


I wonder if the one poster about expansions is confused. We were told not to constantly expand abbrevations anywhere in a report if they are real long. You know, like that SIADH syndrome. Just once maybe in the diagnosis section, that's it. Then abbreviate.

I have put so many of the respository samples on my desk top I do the same thing, copy and paste.

I have hundreds of samples, "standards" templates and do use regularly and will continue to use. Speeds up by day, reaches my quota.

You know what else is not okay?? getting paid CENTS, not dollars per hour PLUS lines over quota and NO OVERTIME, for what we do. THAT IS NOT OKAY.

So okay or not, absolutely any cheat, shortcut, copy/paste, template et cetera, I AM GOING TO USE.

That is one of our best tools, why not? Without making our standards how could we make any money. You do have to be smart enough and be careful and read along, but who cares. It is like taking a hammer away from a carpenter and telling him he can not use it. Silly.

What MQ does have a problem with is MTs inserting standards that are not asked for in order to boost line count or inserting their own expansions without checking it against the audio. Seriously, we are paid on production and since ASR is never going to get better and it will be a cold day in India before we get raises it is up to us to find tricks/tips to help us improve production. Isn't that what MQ wants anyway, increased productivity? Why would they have the Ctrl-E function if you were not supposed to use it?

I attended a DUF on speech recognition where they gave detailed info on how to do this, and explained how this helps the machine "know" what to enter.

We just had a discussion about this with our QA lead on a conference call. What she said was you cannot use a standard NORMAL in CTRL+S as an expansion because the clients can change them at any time and then you won't know whether you have the updated normal in your expansion. Anything else is fair game.

doctor says to use his standard you use it. However, he did not approve the typos within the standard, you have to change those. He has dictated the standard, but it is only as correct as the one who entered it into the list.

QT And Templates

Does anyone know the going rate for qualified text?  Between that and the templates my accounts uses, my line count is down by at least half.  The company I am with now pays 1 cpl for QT and the templates that load require more time to delete headings, cut, paste and move around than the actual typing.  We use Emdat, which is a good platform, but the templates and QT are only a benefit for the client (and company) and certainly not the MT.  I think this is probably worse than ...

I don't know why the templates aren't in line with account specifics--the fonts are all over the place, headings are bolded, words are abbreviated and slashes are used...sometimes there are blatant misspellings. Hi - I have a question regarding counting templates. I have a new doc who wants me to only count what he dictates, and not the templated review of systems, past medical history, social history, etc. which is all unchanged (unless he says to change it). Do you cut and paste your work into a "count" file to get the total, or do you tell the doctor that it has to be included in the count. And what about the times he does make changes to the "templated items". I welcome any suggestions. ...

We are inundated with sloppy dictators, perhaps the ones that can't use VR. We know how much they hate to dictate repetitive ROSs and PEs, thus they love using templates, or paragraphs from previous reports. Who is responsible for adding the templates?  I am finding a lot of errors lately in these when I pull them in.  For instance, spacing.  Several, if not many, are only single spaced between the end of a sentence and the next and only 1 space after a colon.  Yet another thing to slow us down.  Got "dinged" for pulling one in and not changing.  I assumed (yes we all know what happens when you assume).  That will never happen again if I can help it.  Also noticing 1 space between state ...

For some reason when calling up the templates I am inserting my password yet every time it does not hold it, puts something else in and then my report freezes wht my being unable to get out of anything, template, report, etc and have to restart. Doctors Using Their Own Templates Jeopardize Patient Care

I don't know about the rest of you but I have very few male doctors who are careful enough to adequately edit normal templates to meet the patient's particular criteria. It's gets pretty time consuming to do it myself, especially when we are not qualified to do this in every instance. Female doctors almost always attend to these details. NPs, PAs do a much better job than the male docs, but few male docs either care, feel it is their job, or take responsibility for this editing. ...

VR Standards

This is just my observation.  I am presently working on VR and have been for about 1-1/2 months.  On my straight typing accounts, we followed the Book of Style and typed verbatim.  VR is a whole different world.  Where as we had to spell out abbreviations in the impression, plan, preoperative diagnoses, etc., with straight typing, on VR we are told to not expand abbreviations.  We are also told that unless the statement makes no sense whatsoever to send it to QA, b ...

Thanks for all your comments about inserting standards but I am not concerned about mistakes in them at audits.  I am at the point where I don't care if I am fired.  The point I wanted to make is that the quality of the standard from the hospital was horrendous yet obviously acceptable because it comes from them yet we are expected to be 99% accurate under the worst possible circumstances.  If this weren't so pitiful and causing so many MTs so much distress, it would be ...


SR Pay Methods/standards

My hospital is transitioning into SR using APEX. We're being told that they want to base our expect standards on VBC (visual black characters), saying this is "the industry standard".  Are the rest of you on SR/VR paid by VBC, lines, or minutes?  What were the justifications given?  We've been paid for the last 10 years by minutes so aren't thinking this is a good change.  Any feedback would be helpful.  We are a union shop so would love to hear from other ...

When a report requires a standard be used and it is an ASR report, if the MT knows what the standard headings are, isn't it acceptable to just type in the headings in the appropriate places within the ASR, rather than pulling in the standard by Ctrl S?     ...

Can anyone tell me if we are paid for standards brought into reports or only what we add to it?  I did a very long OP note yesterday that had a large standard and quite a bit of dictation along with it and am surprised to see I only received 9.38 lines for it.  Something doesn't seem right, I know I type more than 9.38 lines even without the standard!   ...

Our facilities have many docs who say "insert my standard."  Yesterday I got a new one which was beyond belief.  The format was wrong; the PE was double spaced instead of single.  Some of the headings were capitalized and some were not.  There were spelling errors.  No Murmurs, Rubs or Gallops appeared like so, etc., etc.  Someone somewhere had to produce this PE and this is what the hospital is using as their standard.  Next time I get a piece of crap like thi ...

Anyone else get really annoyed when a doctor says "use my standard" and then proceeds to make half a dozen changes to it?  What they would like us to do is remove part of the dictation, go down to the bottom, get the appropriate text, copy, delete, move it back to the correct place in the report and then, of course, edit it.  Doing this multiple times is not only excessively time consuming but makes it very easy to accidentally delete text.  Just curious if you are all doing this. ...

Last night I did two fairly long duplex studies where the doctor says "use my standard" with a lot of "move this, take that out, insert this..."  Not difficult, but time consuming.  When I went in to look at my line count after my shift, I got a total of 20 lines for the two reports, or 16.48 cents!  What gives with this?  Did I miss the memo, or did they just not tell us?  Next week they'll be asking us to pay them when we actually get work. :o( ...
