Email from HDSM wishing a happy Father's Day to "all the Dad's on the team."

Minor error for unecessary capitalization of dad. -0.25

Minor error for using an appostrophe on a plural. -0.25

So many times this has crossed my mind! We get emails with typo after typo and stupid errors from them and yet they demand perfection?!?

Whatever happened to teach by example?

when there is no 'noun' in front of it. Don't know the new ruling because why buy this book for 75 dollars when I have the 2nd Edition and make 4 cents. I would think that by the QA that Dad in this case would be capitalized, but surely not the apostrophe. Who cares anyway...surely not Dads.

if you can replace the pronoun with a name and it still makes sense, then it needs to be capped. On a side note, MM doesn't use BOS3, and BOS2 is the bottom of the totem pole of references. MM uses the DQS Standards and Transcription Style Guide above all.

mom and dad are only capitalized if you can replace the word with a name and it still make sense. "All the dads" would not be capitalized.

"Informed consent was obtained from mom and dad" Mom and dad would be capitalized because you could replace with "Informed consent was obtained from Mary and John."

"Informed consent was obtained from the mom" Mom would not be capitalized because you cannot say, informed consent was obtained from the Mary.

Does your example, even remotely mean the same thing as the original sentence? Not at all, so I don't know what you are trying to prove. I think we all know if the word "the" is in front of it, it cannot be replaced by a name, and I'm sure all the dads are not named Joe. Really your post wasn't even worth the time to respond, but, alas, I did anyway.

"Remainder is" or "remainder are" is dependent upon "review of systems." As review is singular, it should be "is."

If it was "the remainder of the systems," it would "are."

The word remainder is singular.

I get dinged for leaving out words like the, and others.  I am now understanding that this is not the correct policy, that others are not getting critical error, major error, off one point for this.  So, am I being singled out? Because I thought this was the norm.  I'd like to know so that while I am hanging on every word and will continue to do so, I thought I was not the only one with this...thanks in advance for the update.  ...

Not to add to the litany of complaints about ASR - but it's just so annoying the way whenever the dictator uses the word "period" - as in, "blah blah blah for a prolonged period of time" - ASR puts in a PERIOD and ends the sentence right there.  ARGH. Has anyone at the Q been getting hit by "critical errors" on crazy stuff that has never been an issue BEFORE?   All of the sudden, after no feedback for about a month, no audits, nada, zilch, I got my audit and was given critical feedback for not capitilizing a department! (recovery room, radiology suite, cardiac cath lab...)  AND, in a verbatim report, with a client profile that says specifically, DO NOT ADD A HEADING IF ONE IS NOT DICTATED, for NOT adding a heading!  This makes me nuts ...

I work in a surgery center/clinic in medical transcription. I have never worked in the medical field before, so I'm not sure what is acceptable. We get massive amounts of documentation errors from the nurses in the surgery center and in the clinic. Is this usually acceptable?! Is it normal for a transcriptionist to be required to scan over the chart and correct all of their errors or should this be the responsibility of the nursing staff? It's DRIVING me up a wall!
