Ran into a trauma nurse yesterday this isn't good news - sm

Posted: Mar 31, 2013

Of course we start talking about everything. Well my local hospital surgeons no longer dictate. Their medical students follow them around and are in charge of keeping track of everything and then later going in and entering it in the computer. This is a HUGE group of hospitals and I am sure impacts a lot of us. Also, my local doctors have begun bringing their computers into the room when your in for whatever, and doing the paperwork right there, on your dime or the insurance companies dime, which ever the case may be. So now not only are they not dictating, they are charging the you for their time.

No surprise - anon

My doctor has been doing this for over 2 years now. Also, been reading a lot about scribes and how they like medical students. We are a dinosaur about to be extinct.

At least the MTSOs will be extinct as well! - silver lining

The family practice clinic I go to has not used MTs in years and years. They were either punching something into the computer or dictating right then and there while I'm in the office.

As to surgeries, they used to be my saving grace in terms of having enough work to keep me going. I've since left the business completely, but I do remember days where there was no work except for surgeries.

Also, we have a local day surgery center here and they just use templates. No MTs whatsoever.

So, anyway, that's part of why I got out. It's just going down a spiral....

I just need a few more years - sm

But if they tossed me aside tomorrow, it could be the best thing for me physically and mentally. Then I would be forced to find something else. My problem is I know how I learn and I cannot retain or learn through online courses. The schools are too far to drive and work right now. But I darn sure could survive on U/E while I go back to school and start my bachelor's degree in something.

Be careful what you wish for. Are you absolutely sure you could survive on your UE? - Mine would be less than half

[ In Reply To ..]
of my pay. It's around $150 per week, based on my last 3 quarters. (My state stinks in so many ways.) I know I sure couldn't survive on that.

Providence Hospitals/clinics in Northwest - for 8 years doing their own.

I took a course in college, EMR. The EMR is really cool to work with, but lost of memorizing where everything is in order to be productive.

What bothers me is scribes is only for practicine docs. Should have been for MTs too. In fact, easier for us to learn it and know what to put in the EMR.

Whose dime it is - CCS, CPC

You or the insurer paid for that time before.

That type of physician service, the office visit, is , for the most part, paid on the basis of complexity of care and how much medical decision making is required. Documentation of the service has always been included. The time required for that and for checking labs, writing orders, phoning in a prescription, talking to a consultant, etc., is factored into the reimbursement rate.

You are just seeing it happen in front of you now, where before the doctor would have stepped out a minute or two sooner and scribbled or dictated.

I am not saying that doing it on a computer in front of you makes for a good patient experience. I do not think it does. I am just pointing out that you actually paid for that time all along.

First EHR, then ILP, now Health Scribes... - sm

[ In Reply To ..]
My hospital system uses Epic and medical health scribes. I've been told by both my daughter's doctor and by an instructor at my community college that health scribes are the "new" thing that a lot of facilities are going to. In fact, when I took my son to the ER a number of months ago, he had a health scribe doing his dictation as he was examining my son. I found it super awkward, though luckily it was just for strep throat. Personally, I don't like having anyone in the room but the doctor when I'm being examined.

You can tell them to leave - sm

[ In Reply To ..]
Like when they bring in a med student they ask if you mind them in there..Tell them you don't want them in there and they will move them out of the room..Of course, its like sending a bad meal back at the restaurant, never know what is coming out of the kitchen after that..

doctors trancribing own reports - msdaisy

[ In Reply To ..]
We have a local ad for a "physician's scribe" where the med student or PA or NP folllows the doc with a laptop and scribes right then and there. What you said in the previous post is right on.

My question - Not giving up

[ In Reply To ..]
My question is who fixes the messed up report they create? My facility's experience with do-it-yourself dictation and transcription has not been good and the outsource company we use is even worse. We've had far too many errors and the providers do not have time to clean it up. They let all our MTs go and are now bringing people back to edit and do QA. The truth of the matter is, many PAs and NPs never learned how to spell and can't be bothered with grammar. Not everything is point and click, language specialists are still needed. This is just as ludicrous as thinking that all coders will be replaced because of computer assisted coding. What goes around, comes around.

I don't think we'll all be replaced either, but... - sm

as we have already experienced, pay is going down, down, down, and an actual good MT job is a thing of the past. By good, I mean, good pay and actual work to do, not just sitting around twiddling our thumbs waiting for work to come in. That's not even a job, that's just insanity. Soon, there will be very few MTs needed. Personally, I think being an MT could be a good part-time job to make a little extra pocket money, but as far as it being a solid, secure full-time job, or a career? Um, no, not even close. Sad but true. I feel like I've been in mourning over this for so long and am just now starting to get closure on it. I love being an MT, and I miss it.

So Does Yesterday's News Make Anyone Think We Are Getting A Raise
Oct 09, 2012

I Got A Good Laugh Yesterday
Nov 09, 2011

I got an e-mail encouraging me to test on-line for MQ and submit my resume and they would be anxiously awaiting it and monitoring their system for it to come through so that they could promptly contact me about a position.  I do have a lot of experience in all work types, lots of ESL experience, and my QA reviews have been high, so I know I would be an asset.  I also know from posts over the years that MQ has some huge issues, and I don't see that getting any better. I got an e-m ...

Yesterday Was My Last Day,feels So Good To Be Free
Apr 15, 2014

After 20 years at MM and 5 years transcribing before that, I'm officially retired.  MM was a great job when I started. It allowed me to stay home with my kids when they were small and I even made a good wage but lately...this company has gone to the dogs and I am so happy to be done with it.  There was a time when we were professionals not just typists working for peanuts. I will miss transcribing but I will not miss MM. Au revoir MM.  ...

Possible Good News
Mar 19, 2014

I interviewed for a job at a local, major hospital.  It was interesting to learn that the HIM department is in the process of bringing transcription back in house, at least partially.  It was claimed that their transcription company was not doing a satisfactory job.  This might be good news my MT friends.  Maybe this will be a trend?   I'm certainly hoping so and I mentioned how excited I was about the possibility of in-house transcription jobs being available in th ...

Any Good News For The Future Of MT?
Dec 07, 2009

I've been following the MT boards and what I mostly read are MTs complaining about losing jobs, lower pay scales, outsourcing, offshoring, voice recognition, electronic records, and other technology enhancements that seem to be putting MTs out of work.  Is there any future for the MT profession?  Is there any good news or is this profession going the way of the buggy whip?  I would certainly like to read some good news about the future prospects of MT. Thanks Hopeful in Hob ...

Hopefully Good News For The MT World!!
Mar 31, 2011

So I was at a local clinic today that had switched over to a dropdown menu system and templates for their doctors a couple years ago and had laid off their transcriptionists and the doctor was just typing away in his template and of course I had to mention how as a medical transcriptionist this really bothered me.  He told me that many of the doctors there were unhappy with the new system and had gone back to dictating and that they were more than likely going to be hiring transcriptionists ...

Good News For M-Modalers

If you received an E-mail from your TCM this weekend about how you are expected to be logged in for your entire shift every single day, no more, no less, print out that e-mail right away and save it! Do not delay! Be sure to log into your time sheet on time and be sure you log in your 8 hours (or whatever your shift is) and also stay logged on your network whether there is work or not, and get a print out from the network at the end of the workday to document that you were logged in the enti ...

Nuance What's The Good News?

I really appreciate all the replies, good and bad.  I guess the good new is we the displaced have time on our sides in that we have 58 days to make a decision about our future employment, I believe the union did get us that.  I'm definitely going to look at other companies, possibly independent contractor positions.  There are some leaving who have kids/families they are supporting, I am 59, only support myself so I have that at least.  Appreciate MTStars board. ...

Nuance - What's The Good News?
Apr 11, 2013

We got the news yesterday, offered jobs at Nuance doing our same doctors.  I understand we have to clock in, and will now have to pay for insurance, internet not reimbursed, less pay, very large company.  We work on Escription with small amount of straight typing. They say will transfer our seniority, etc.  I have read on this board in the past that thngs may start out well but don't neccesarrily stay that way, things go downhill.  I would love hear from someone who works there and like ...

Keystrokes - Any Good News On Them?
May 07, 2013

I've actually worked for them in the past, horrible experience with my team lead - who sucked, and my account, which was an overflow account - which they didn't tell me when they hired me.  So needless to say, I sat most of the time without work.  So I didn't last long there, as we obviously need work to make money.  I'm looking for an employee position, as I'm sick of owing taxes as an IC.  Do they have straight typing there or is it mostly VR?   ...

Good News! Smartype Is Back!
Feb 18, 2012

I'm so happy there is a new version of Smartype that has been released. It now works with newer versions of Word, i.e. Word 2007 and 2010 and works with other software including Outlook. It's even better than the original. I was using the old version of Smartype even though it had glitches when working with Word 2003. You can try it before you buy it, a definite plus. I highly recommend downloading this software. To me, it is a must for the medical transcriptionist to boost productivit ...

For Those Of You Thinking About Becoming A Nurse

I just heard tonight from a lady who has been in school studying to become a nurse. She told me basically her schooling is now over. She made a 74.? in pharmacology and you have to have at least 75. Which Course For Experienced Nurse
Aug 11, 2010

I am looking into MT programs and wondering about one which advertises that it is geared to nurses: MT@Home Medical Transcription. It costs significantly less than the three programs which I see referenced quite a bit  -Andrews, M-Tec and Career Step, but it seems like these programs involve so much material that I already know. Years ago I did transcription for nurses, I've worked in hopitals/home and office environments as a nurse (also years ago) and I worked in pharmaceutical sales ...

To The Nurse Practitioner...
Feb 21, 2013

Just wanted to dedicate this post to the nurse practitioner who coughed very LOUDLY throughout two 10-minute reports I just did. Just to let you know, there is such a thing called the MUTE BUTTON. Thank you so much for just about blowing my ear drums out. I literally had to pull my ear plugs out so you wouldn't render me deaf. I feel sorry for whoever uses that dirty, germ-filled phone after you hacked a lung into it. Oh, and BTW, the phrase "carotid bruits" is not pronounced carotid brew- ...

Nurse Medical Transcriptionists ??
Jun 26, 2011

I came across an ad recently catering to unemployed new registered nurses who need jobs badly. I am understanding that the market for RNs and healthcare in general is saturated and then some; however, this article specifically stated "Nurse Medical Transcriptionists." I saw it on All Nurses forum; I don't remember the company . . . BUT a lot of nurses were responding with interest because they can't get jobs. The only thing that was running through my mind was "you have no idea people ...

RHIT Or Registered Nurse
Apr 15, 2012

Just wondering what the majority thinks about these career paths. It seems as though a lot of MTs are moving or considering a move  to the RHIT/RHIA world of especially medical coding. I am assuming that the lure of 'work at home' is pulling many in this direction, and I have actually seen where many are questioning whether medical coders are 'paid per code' or 'paid per case' or what? I asked myself what are these people thinking? Has anyone experienced this, ...

Registered Nurse Wants To Break Into MT Field
Jan 18, 2010

I have been working in this field for many, many years.  When I started, it was a little easier to break into this field.  It was in an era when everyone didn't work at home and every company didn't require 2 years experience.  I figured the newbies on this board would know the answer to my question.  I have a friend who has been an RN for years and would like to break into the MT field.  Being that she has terminology/medical experience, is there any way ...
