QA feedback? - Woe is me

I got feedback on a report today, CRITICAL FEEDBACK.  The doctor dictating was absolutely horrible, worse than horrible. I could not understand him, tried my hardest and before I uploaded it sent a note to QA asking for a sample of his work, explained that I could NOT UNDERSTAND HIM. 

In reviewing the revisions, I don't agree with many of them.  I still don't hear what the QA inserted.  I dont' agree with it and I'm angry that it was returned to me with CRITICAL FEEDBACK.  I called the supe and was told not to worry about it, yet that put me at 96% accuracy and you know how they frown on that.

Anyone have that problem? 

QCs are the only ones who score reports on audits, not QAs. - nm

okay... insert QC in place of QA above, same story. Cannot understand him, don't agree with the corrections, still cannot hear what was said,think getting critical feedback on a report that is inaudible is not right.

Same here, critical error, terrible ASR/ESL crap, and 97.8% average accuracy for one week's work. The only answer to this problem is to send everything I cannot understand to QA. They give me no choice :-(

This is my opinion:

You know what they're doing don't you? They audit the worst of the worst reports with poor sound quality, background noise, muffled voices of the worst dictators. I too got critical feedback on stuff I couldn't understand. I also don't agree with the audit results.

I make 10 cents a line. They're trying to get rid of those of us who make the highest line rates. They can hire 2 newbies for less than what they pay me and have them work third shift.

By scoring us so we fail our audits, they don't have to pay the monthly bonus (which is a joke anyway), and they'll get rid of those of us old timers who get PTO at a higher rate. If we fail audits and get fired, then we don't get unemployment. They're pond scum.

Absolute craziness. I also have been here forever and make 10 cents per line, and failed my first audit ever! They marked me wrong, critical error, for not sending a report to some department of the hospital or to the dictator (even though dictator automatically receives a copy). What is with that? I also noticed that 5 other reports were 100% but 1 was marked at 81%, 1-1/2 points marked off for the cc's and 1 point for a word after a QA marker being wrong. So I failed.

They will do what they do. My 2nd job I have been 99% and 100% every month for the past year and a half. It is not me, but I really don't care anymore.

give us her thoughts on what they're doing & why. They've drastically upped the audits on US when it's the ILPs bringing down the quality and causing the client complaints. Doesn't really make much sense to me.

My QC used to be nice, funny, now she is a witch. I think they are all miserable, they all know what is coming, they are seeing the ILP work and those audits, yes, got one too, always 99%-100% before, are to cover their A@@ when they no longer need us. They have to lie to us to keep their jobs and that cannot be easy. Plus so many MMEs quitting that I am sure they are stressed to the max. Lovely company we work for.

I know my main accout has stipulated no ILP MTs- even the Canadian MTs got booted off this account- but it makes me wonder if my QC is being extra picky because MQ is actually trying to lose the account??? How easy for them to get the client to break the contract when the MTs do such a "lousy" job!

Also as a heads up, when the QC grades your reports she has to do a full listen and that "score" that shows up in QASAR is not a true audit, but don't think your CCM/QC aren't keeping track of all your report scores to throw in your face when you God forbid ask for a raise or to move to an account that actually has work. It is a lose/lose for the MTs every time.

The coaches are performing audits on random reports before they go to the clients. In the predelivery stage, they do not know what is coming to them. They are the only ones who can score in QASAR, and it is done according to the quality program for the company. The reports are reviewed 100% to audio. The highest number of errors seem to occur in ASR reports--nots of corrections not being made. Auditing will occur on a regular basis. The results should cause the coaches to work with any MT not scoring 98%. The audit should be conducted on several reports at a time, not just one report.

QCs are not upping the audits on US MTs only. We have to audit EVERYONE, every single month now. ILPs are also getting audited and are held to the same standard (so we are told, anyway). We must audit 1200 lines a day, from live QA or by having MT work routed to us if QA is caught up. Try not to let it bother you too much if you are getting lots of scored reports, because actually, the more reports the QC scores, the higher your audit score is likely to be.

Now, regarding the "critical" feedback - the label "critical," "major" and "minor" are all programmed in QASAR. We can't change it if QASAR calls an omitted dictation critical. We can, however, downgrade it to 0.25 points rather than 3. Try to look at the point value rather than the adjective that QASAR is giving it, and go a little easier on yourselves (and your QCs). Most MTs do a great job, and I can tell you that I personally try to always downgrade errors if possible!

I just started recently with MQ but since being here, I have had 2 critical errors and 1 just a step below that and I don't understand it. At my last job I never had a critical error once, ever, even during training. I can't remember off-hand what the first critical one was, but the one from today was that I left out a word. When I looked at it, I had left a marker and QA had filled it in. Obviously, I didn't leave it out - I couldn't hear it and I left it marked appropriately. Not sweating it too much right now though. Just glad I have a place to come and see I am not the only one with questions.

Wow am so glad to have found so many others that are having the same problems. I have been a transcriptionist for 20 years,over 10 now with MQ and first time ever I failed a quality audit 3 weeks ago. Was reaudited today and see I failed again!!! The only reports audited were the difficult dictators that you cant understand nor hear. So what happens now that I failed two audits? Am I going to lose my job? Have been sick to death over this all day, just waiting to hear from my QC or whatever her title is now!!!

I'm so happy!  I got my audit, passed with flying colors and got an email from sup saying good work and keep it up!  He also suggested that I submit for a raise since I've been consistent.  I said I didn't know that we could, but apparently we can if we maintain that 98% average consistently for a certain amount of time.  I'm submitting my request as it has been over a year that I have maintained this.  Wow! learn something new every day with the Q!& ...

I started with MModal a month or so ago and I am noticing that MME feedback is not consistent.  One will cross out dictation in the document that another will leave (like service or surgeon or primary doc). Also,  "myself/himself/herself" should not be changed?   I have to admit that one was a shock.  It is just soooooo wrong.  ...

Would appreciate feedback from anyone using M-Modal, especially Webmedx Enterprise MTs. Dreading the conversion and wondering if my fears are well founded. I'm pretty fast straight typing but rarely get more than 50 lines more an hour with current VR. Seems all it takes is just one lousy report to negate the lines made on the decent reports. Afraid it's just going to be worse with M-Modal and the whole drop-down heading thing. Anyone have any encouragement to offer? ...

Have a hospital where you are to phonetically spell name if not given (it wasn't except to call him "Steven")  QA so kindly sent me QA feedback and the only thing on there was that apparently Steven was actually spelled "Stephun."  Is that ACTUALLY going to be of any help in the future?  QA wonders why we have an attitude?  DON'T WASTE ANY MORE OF MY TIME THAN IS ALREADY BEING WASTED.  ...

I just started working 3 days ago and have been doing very well with QA (100%) until yesterday. It seems now I am getting more scores in the high 90s, but I really feel that some of them are error of the QA. I got dinged for this: Blood sugars were in the 150-250 range. Most of them, though, were around 200. QA took out the commas around "though." Would you do that??? I figure "though" is nonessential and is extra info, hence the comma.
