Bowing to Physician's wishes - WorkingMT

Posted: Jun 24, 2011

Just got an E-mail from our QA person stating that a physician is complaining that we are not sending CC's to the PCP that he gives at the beginning of the report.  Now, I do realize that this information has been placed into the Client Profile under specific instructions for this physician, but sometimes when you are working on the same facility and feel comfortable you stop looking at the CP for every single MD.  This is not a facility I type on often, and apparently it is not just me.

Why is it we have to bend over backwards for these physicians, why doesn't someone just say to the guy, "you know if you would just state send cc to PCP" the problem would be solved, but NOOOO he has to write a formal letter and complain about this.

Solve your own problem, open you mouth and state one more sentence.  whew, okay done ranting.

I just don't understand why it cannot be explained to these MDs that the MTs that are doing their dictation work for many other facilities and that the company-wide policy is that no cc is to be sent UNLESS THE MD STATES BECAUSE OF HIPPA RULES-----and that if they want a cc sent to BE SURE TO DICTATE THIS AS IT IS THE COMPANIES POLICY.

Hello, they must be aware of HIPPA laws violations, why does all this have to fall on our shoulders?

My final thought on this subject: I used to work in-house in Medical Records, was a Chart analyst and Transcriptionist; it was always the chart analyst's job or the medical secretary's job to distribute copies, never the transcriptionist's responsibility. Why or why is it not the facility's Medical Records Department or better yet the dictating MDs secretary's responsibility to make sure that these copies are distributed when putting together charts in their offices? Why is it a remote virtual employees' responsibility???
Because as bottom-of-totem-pole, invisible workers, we - are the easiest to blame for EVERYTHING. n/m

Seems to me that rather than rant, you can solve your own problem by putting a bright post it note up on your bulletin board or computer wall... We ARE suppose to check CP ya know.

As far as the "it used to be"... it's not anymore. It's a whole nother can o worms and we've just got to roll with the punches or not. Your choice.

Here's the deal on cc - sm

With the consequences extremely DIRE for the lowly MT/MME if the cc is sent to the wrong person, not to mention the fact that this is a service we provide for FREE, we were instructed by our CCM we were only to send a cc if the doc specifically said the words, SEND A CC TO.... This does not include saying "the PCP is ..." We have to have explicit instructions to SEND A cc. And, the doc must give the FIRST AND LAST name of the intended recipient.

IF we are at all unsure of the cc request, we are to leave a QA note to the facility requesting they clarify the request.

Best to NOT cc and have a miffed doc than get fined and/or FIRED for sending a cc to the wrong person.

Wow, this board never ceases to amaze me. I just needed to let off a little steam. As the poster just recently stated in her post, we have been instructed by our CCMs/Company to NOT send a CC to anyone unless the physician specifically states to do so and if it is not clear to do a post-it note letting the facility know that the information was not sufficient enough to confirm the cc.

All I am trying to say is that why do we have to bend over backwards for 1 physician out of 1000s. All he has to do is say, please give a copy to the PCP mentioned at the beginning of the report. REALLY, what is so hard about asking a physician to change instead of us?

And YES I already have put a BRIGHT YELLOW post-it note up on my computer even before I made my rant, yes I do look at the CP but sometimes you can read it and still miss things as they are very confusing and not always that clear.
youre right, we bend over way too much as it is - ...

I have also heard this from him in one of his reports: "Please type the proper wording. Don't type whatever you like to type. If you do not understand. Do not type it."

Trust me doctor. If I typed "whatever" I liked to type, you would get an earful (or is that eyeful?)!

Well Wishes
Feb 09, 2012

I am not a religious person so I didn't want to be hypocritical and go on the prayers request board; hence, I am here to get hopeful wishes and good vibes sent my way.  I'm trying to get out of the MT field (like many others).  Thus far, I've had no luck returning to my prior career field and am concerned that no one wants to give me the chance because I've been out of it for so long.  Which, really I don't blame them.  I came close to getting one job ...

Resident Physician
Nov 26, 2010

I have alway been told when in doubt, leave a blank. Resident did not give attending physician's name. There was a doctor's name in the hearder as attending, but I  was not sure so I left the attending line blank at the bottom of the report. My boss calls me and tell s me to go on a limb and guess in the future.  I do not like those ethics because it leaves me up for problems in the future.  Comments welcomed. ...

Associated Or Associative Physician
Sep 14, 2012

The doctor I transcribe for sometimes says "Associated Physician" and sometimes says "Associative Physician" and then lists the doctor's name.   Sometimes he will also list a "Primary Physician."  Just curious to see if there is a difference between associated vs associative and if one would be better than the other in this situation.  Any thoughts? Another Physician's Comments On Using The EMR
Nov 23, 2012

Below is a link to a NY Times editorial.  "The advent of electronic medical records has been a boon to patient safety and physician efficiency in many ways. But it has also brought with it a slew of “timesaving” tricks that have had some unintended consequences. These tricks make it so easy for doctors to document the results of standard exams and conversations with patients that it appears more and more of them are being documented without ever having happened in the first pla ...

I admit, this is a huge pet peeve of mine.  The consulting physician is the one who goes to see the patient at the request of the REFERRING physician.  I wish I could get some docs to read this and state the difference.  If I was typing, I could just correct it right away but I have to correct it on the VR.  ...

Just A Message From A Physician
May 21, 2015

Hello everyone and sorry to intrude. I just wanted to say thank you. I'm a hospitalist physician and I dictate daily. I would say about 10% of my dictations contain a "_______" that needs to be corrected, maybe less...and I am someone who mutters a LOT. So I'm surprised it's not a much higher percentage. And when I hear some of my fellow internationally-born docs dictate, I'm amazed that ANYONE, let alone someone over the phone, would be able to understand what this person i ...

Physician Do Not Dictate List
Mar 15, 2010

Not Sure What To Do. Afraid Of What A Physician Is Asking Me To Do. Advice
