it is sickening to have an RN who dictates for MULTIPLE doctors and she has no idea how to pronounce many of the words, cuts her sentences off, flies through the dictation and has no idea how to pronounce medications. She pronounces tachypnea (eash enough word, right?) as tacky P nea, no joke. I am frustrated, angry, fed up and ready to throw my computer out in the yard. This is just sickening!!

Just needed to vent and wished I felt better but sadly I do not because I still have MANY more jobs to do that are HER!!!!

Probably not the same one, but I get so crazy when she dictates. The doctors this one dictates for are bad enough, as you say, and I was so happy when they brought this nurse in to dictate, but she is WORSE than they are. And takes longer and can't pronounce JACK SH*T!
I want to reach through the phone and beat her over the head with it!! And she sounds like an aging Valley Girl, all nasal and whiny.

I went through three ergonomic keyboards and threw my books at the wall out of frustration. That's not the answer. Look for another job. Believe me it can get very expensive. The companies are not worth what they are paying you.

What makes most "proxy" dictators so awful: - Briana

1. Most of them are paid by the dictation - for instance, $25 per discharge summary. They can make a pretty good buck - but much less if they do a good job of analyzing the chart before they dictate.

2. Usually, they have never seen the patient, were not involved in their care, etc., and usually have no particular experience in each of the various specialties that might have been called in on the case, the medications, tests and diagnostic procedures used in those specialties, etc.

3. They're usually not hired for their breadth of experience because none is considered to be required. It's "just a discharge summary, right?". Anyone with a bit of medical knowledge can read some drivel off the chart, right?

It's just my opinion - and nothing for which I have the slightest proof - but I believe that not a few of these "proxy dictators" are:

1. Marking time between clinical jobs.

2. Not able to obtain clinical positions for professional reasons.

3. Might be "impaired" in certain ways. What better place to park a health provider who has a problem, such as substance abuse, etc?

Just a guess, mind you, but it comes from listening to an awful lot of these folks and observing the poor quality of their work.

We're supposed to be the medical language specialists, or so I thought.

We're all supposed to recognize proper pronunciations and uses of medical terminology.

We're all supposed to be able to recognize nonsensical or erroneous dictation.

We're all supposed to be able to spot logical errors in the document (e.g., contradictions in differing parts of the document).

I'd call that "expert", and if you yourself aren't an expert, what the heck are you doing here?

My husband has a friend who is a doctor who I happened to have the opportunity to talk about dictation with. First I asked him if dictators were ever given any formal training. He said none whatsoever, they have to do it to provide documentation SO THAT THE HOSPITAL GETS PAID. I told him I thought it was for the benefit of the patient and having a correct medical record to which he told me that really had no bearing, it was all about documentation for money. Its all about the money people. I asked him why they were so terrible when they dictate, fast, etc. He said most dictators, especially residents, have no time, they are rushing constantly all day and just try to fit it in to DOCUMENT so they get paid. Its so sad, but that is why we have to deal with the crappy dictations. They do not care, do not have to care, they just have to do it.

Just wondering why there are so many doctors/dictators with foreign accents here? Most of mine seem to be from India. My BIL is a doctor and told me years ago that "smart students" aren't going into medicine now because the average salary is "only" about $100,000.00 a year.  He said the top students are going into business (MBA degree, etc)  in order to make more money.  Aren't doctors supposed to go into medicine mainly because they want to help people?  Can' ...

A new ESL dictating a DS. Can't speed him up because he's too "new" (to this country?). He is spelling every other word!!!!!! Recently the only work availabe has been the absolutely worst dictator I have ever done in my 25 years transcribing for hundreds of different doctors and specialties. Has anyone ever considered coordinating with MT coworkers to refuse doing jobs for horrid dictators? Does anyone think this might motivate the MTSOs to have the backbone to contact the doctor to try to resolve the problem?
