I've seen reports and mistakes that I know I didn't make as well. I know it takes time out of production, but I keep a log of all job #s that I transcribe/edit so I can check to make sure that I actually did that report. I know you can track the jobs in the platform, but I still always, always keep track of job #s. That way you can tell if you actually did the file or not. I would ask for a reversal either way. It's worth a try, but don't hold your breath on it getting reversed. I got 100% QA a lot of times before the crash but once we went back on the grid after the crash I've only got 100 a few times and that was only because for some lucky reason there were no audits on my files that week:) Otherwise always low enough to take a chunk out of my pay. There are only 1 or 2 files checked a week and lo and behold there's always an error that takes my percentage down. Since being back on the grid I haven't gotten one post audit file with a score over 96-97, not one! Something's very wrong with this picture.
