When typing combination medications should you put the dosage abbreviation after each medicaotin..... Example: hydrocodone 5 mg/325 mg or hydrocodone 5/325 mg????
Also, what if it is a medication combination that uses both mg and mcg? Example: 75 mg/200 mcg. I received a correction back from my boss and she had it 75/200 mg and, considering that part of the medication is mcg, i think that her correction is actually incorrect.

actually you are not supposed to use the virgule - dangerous abbreviation list

5 mg per 325 mg (if the mg is dictated)
or
5 per 325 (if the mg/mcg is not dictated)
The reason is that the virgule could be misread as the number one.
IF the medication is trademarked with the virgule, then you maintain the virgule in the statement.
5/325 (or whatever the dose for the trademarked medication)

[ In Reply To ..]I agree with your thinking. My opinion is that it would be better served without mentioning mg or mcg in conjunction with the doses (just leave it 75/200), but sometimes you have to just go along with what your supervisor wants.
One time I worked onsite at a hospital, but I was employed by the transcription service. One day my supervisor came in and saw I had listed a patient's allergy as SULFA. She told me that was wrong and the word that was correct was SULFUR (as in the chemical element that is present in foods). I tried to explain what sulfasalazine was, but she insisted the correct word was SULFUR. So the day she was there, I used her word. When she was not there, I used SULFA.
Point being, do what the person who signs your paycheck tells you to do even if you think it is wrong. Just keep her feedback for documentation should something come back around on it.

This case drove me crazy because the doctor dictated "mg", but the medication had both, so it's almost as if i should transcribe it with both mg and mcg or not use them at all.

[ In Reply To ..]You had Arthrotec correct as 75 mg/200 mcg. Your boss used mg for both numbers so it should have been 75/0.2 mg (which equals 200 mcg). I think she overdosed the patient.

I am not sure how to format this medication and I think there might be a stumble also. Doctor says "... 2 tabs 125 mg 2 times p.o. t.i.d." Wonder whether I should edit out the either "2 tabs" or "2 times" as she does seem to correct herself. Any thoughts on this would be appreciated? Thanks ...
