preoperative diagnoses format - jt

I know when there are more than 1 diagnoses, you list each one separately.  How do you handle it when the dictator gives 4 separate diagnoses, then "etiology to be determined" at the end?  List everything under 1 dx, or list "etiology to be determined" as a separate dx?

that's tough because he could mean the etiology is undetermined for the last diagnosis in the list...or he could mean the etiology is undetermined for all of the diagnoses. ugh.

was there a clear pause between the last diagnosis and the "etiology to be determined"? clear enough that you could assume he means it for all of the diagnoses? or was he quick about saying it right after the last diagnosis?

i would probably just make "etiology to be determined" the last diagnosis in the list, if it's not completely clear that he means to say that specifically for the last diagnosis in the list.

Judgement call, for sure. If he said "etiology to be -sm - Idle MT

determined" very close to the last sentence of diagnosis #4, I would put it there.

If it sounded like an entirely separate thought or sentence, I would put it as a separate sentence at the bottom of all 4 diagnoses, without a number, and the reader could associate it with whichever diagnosis he/she preferred, yet it would still be closest to the last Dx, "just in case".

I'm having a problem with this part of the diagnosis as she doesn't specify to go on to the next number, as she does with the rest of them, but I've never had someone that adds the procedures used to fix the diagnoses in this section.  This is the sentence as she says it, "1. When typing combination medications should you put the dosage abbreviation after each medicaotin..... Example:  hydrocodone 5 mg/325 mg   or    hydrocodone 5/325 mg???? I am doing a DS.  I can't believe this has never come up before (or I've forgotten!) - When typing LAVH-BSO on the diagnoses, can I use LAVH-BSO or do I need to spell it all out?  I would think LAVH-BSO would be more readable, but I know the rule is to spell it out.  I'm trying to be practical here, but it may come back to haunt me. Why the constant slang and abbreviations in diagnoses and impressions?  Especially when it could mean more than one thing ( for example I and D) and it cannot be gleaned from the record.  Are these clinicians lazy or what. I think there should be stricter guidelines for those dictating, rather than placing the call on the MT.  I know I am preaching to the choir so to speak, but it is really getting on my nerves... ...

I am not sure how to format this medication and I think there might be a stumble also.  Doctor says  "... 2 tabs 125 mg 2 times p.o. t.i.d."   Wonder whether I should edit out the either "2 tabs" or "2 times" as she does seem to correct herself.   Any thoughts on this would be appreciated?
