Is this right?

Mom is a G2, para 0-0-1-0

Aren't we missing a number somewhere in the para section? Should I leave a blank somewhere?

She is currently pregnant - Janie

Is the patient currently pregnant?  That is how they come up with the gravida 2.

The mom was 4 days postpartum, so she couldn't be pregnant, but the doc was probably reading off her history from an old note. I would imagine it probably was supposed to say G2, P-1-0-1-1. I just left it blank.

Don’t you just hate it when they do not think before dictating?  If he/she had just dictated that the patient had just given birth, it should have been obvious that G2, para 0-0-1-0 was not right.


But, come to think of it, I have heard them dictate “previously” G2, para 0-0-1-0 for new births, just to cover themselves, especially if there are any complications and if the baby is in distress; or they are not sure about preterm status. 


Hope your QA has the sense to know why you left the blank.  That is another pet peeve of mine – leaving a blank because of inconsistencies and then having QA go ballistic because “it was clearly dictated and we type verbatim.”  Duh – wrong – is wrong – even if it is clearly dictated.

Luckly I work at a small MTSO that is very close knit and undstanding. We don't really have a QA in that sense, up to 2 blanks go straight to the doctor, and more than that is edited/QA'ed by the in house MT department, so we are all a happy family ;-)

Thanks for your help, I would not not thought about that without you suggesting she was pregnant (which she WAS - last week!)
