Typing a physical and patient has allergy to _______? It sounds like E-nax-e......any ideas?

not enough information... possibly Ranexa - need more info.

...no other clue with the rest of the note other than she is 54 and only other RX is Vivelle-Dot 0.05 mg patch. she had kidney stones, pneumothorax, prolapse since birth, osteopenia, and has been off Boniva. Had a hysterectomy for uterine fibroids and endometriosis. Doctor recently prescribed Macrobid

With an allergy, if not absolutely sure, I would leave a blank...nm - brrrr

Hi, I'm transcribing a doctor's note that's describing the patient's allergies and how the patient has tried various OTC drugs, including 'pseudometafecin' but for the life of me, I can't figure out how to spell that correctly; I've tried all of the different variations that I can think of (sudometofecin, pseudometaphecin, etc.) and tried looking up 'allergy medications' and coming up with squat. I have a feeling I'm very close to how it may ...

I am on a verbatum account. Doc say past medical history is unremarkle. Then gave meds. Then listed as the first allergy hayfever. In your opinions, would you list hayfever as an allergy as it's verbatum, or would you put hay fever in past medical history. I've never known hay fever to be an allergy. Thanks, M ...
