Quick help needed on punctuation. - Rainbo

Unsure of comma placement/procedures. It is all running together in dictation. I'm sure of the dictation, that part is clear. Thanks! Procedure is posterior arthrodesis at L4-L5 bilaterally, posterior non-segmental instrumentation in the form of pedicle screws with rods, fluoroscopic guidance and supervision and interpretation and placement of percutaneous pedicle screws, greater than usual procedural services secondary to revision nature and the claimant being graded at 100 pounds over the ideal weight surgical assistant. Plan states procedure in the postop period: Use of BMP with preparation and application.

last line doesn't make sense, with or without punctuation. - sm

I am not too nitpicky about punctuation in the "procedures performed" section. You have to understand that the claimant is 100 pounds over the ideal weight. The surgical assistant is always identified in a separate heading; maybe the name sounds like "Plan states." Then you lose me. It looks like "use of BMP with preparation and application" should go back under the "procedures performed" heading. Are you sure you heard "postop period" correctly?

The last line didn't make sense to me, either. I posted exactly what she said. She didn't pause or make anything clearly separate from anything else. It was very confusing.

This doctor is testing proverbs on this patient. He does a lot of I said, he said stuff. I'm not sure how exactly to punctuate this. How would you do it? How do you punctuate "however" when docs and nurses dictate it more like a subordinating conjunction rather than a conjunctive adverb?  In other words, they use "however" the same way one would use "yet."  An example:  The patient no-showed his last 3 appointments however showed up for this one.  (I purposely left the punctuation out.) I know how to punctuate it if the sentence reads:  "The patient no-showed his last 3 appointments; however, he showed up for this one." ...

Why do they make such a big deal out of an error, including sending e-mails saying "ERROR ALERT" or making a 3-point error a "critical patient safety error," then docking your pay for same, but if you go back to look at the document or to listen to it again, which you can in Escription, they have NEVER CORRECTED THE ERROR??? I don't think they are that concerned about errors, other than to dock our pay and to shame us. If they were, they would correct the error in the original documen ...
