Under the physical exam,
Derm: Dry, scaly psoriatic s/l leavings on the bilateral fingers.
Nails: No pitting. Possible s/l onicolisis on the toes. No capillary engorgement.

The only jobs that were actualy transcription for me today were the worst 4 doctors on the planet! This dr. is dictating some sort of cream that s/l triensemalom cream. This is the second patient she has dictated this on. At first I defaulted it to Tridesilon cream, but it really doesn't sound like that. The patient was also prescribed Clobetasol cream. She has psoriatic arthritis, OA, DDD, fibromyalgia, carpal tunnel, diabetes, and hypercalcemia. She is also on Humira, Plavix, nabumetone, oxycodone, Ultram, and Fentanyl and Lidoderm patches. When I start typing for the first time every day, it seems like my fingers are not as fast as my brain and want to get "tangled" because they cannot keep up.
