Help!! Thoracic pedicle screw - s/l LEXI

Oriental neurosurgeon, really butchering this report, says this screw about 100 times, sure s/l 4.0 x 30-mm Lexi screw. Can't find. Please please help!!!! TIA
The doc is doing a mesh placement for a enterocele and I cannot tell which he is saying.
"The pinnacle/pedicle posterior mesh was then brought out. The right arm was then loaded onto the Capio needle driver."
"The left arm of the posterior pinnacle/pedicle was incorporated into the Capio needle driver and likewise placed over the sacrospinous ligament." ...
How are the thoracic vertebrae listed if dictated individually, i.e. T 8, 9, 10 and 11, or T8, T9, T10 and T11? In other words, should the "T" be added before each numeral? Thanks. ...

Looks like the MTSOs have found a new trick pony. They tried to screw Medical Transcriptionists out of hard-earned money by trying to implement line counts based on VBC (visible black character), but very few MTs were willing to accept that, so not many MTSOs are still trying to push it.
