Coronary Bypass Operative - Mary triplet mommy

A reverse saphenous vein graft was then constructed to the third obtuse marginal, brought to the circumflex using running 7-0 Prolene for old distal anastomosis and probing old distal anastomosis prior to completion of _______ (s/l    d-cert they were paytent??).  Cold blood cardioplegia was given through each vein graft with the completion of the distal anastomoses.

I have hilighted the areas that I need help with.  Any suggestions??

All I can offer is this: 


the third obtuse marginal, brought to the circumflex = the third obtuse marginal branch of the circumflex


That other word is "patent", which means open.


I don't think constructed is correct, but I have nothing to offer in its place.  I don't think "old" distal anastomosis is correct, but I don't know what you're hearing.  


It's good to get familiar with the anatomy and the subject, it reduces the guess work.

cert. should be circ., which is short for circumflex. Paytent is patent. And it should be anastamosis. Hope this helps!

I think that the "cert" sound is more likely to represent "prior to completion of insertion", as we are discussing anastomosis.
