Commas or no commas in this sentence - sm

Dr. Hunkyman placed the bronchoscope and noticed an endobronchial lesion which, on fresh frozen section, looked like small cell cancer.

Or should it be:  which on fresh frozen section looked like.....

if I were personally transcribing this report, I would put the commas in.

I commonly see sentences that'll go 3 lines with nary a comma in sight. I once commented to (but did not take points off on) an MT about her propensity for just such a thing. She got all huffy and wrote back, "I do my commas just fine THANK YOU!" LOL.

I would put the commas before which and no where else... - QA too

I was taught that a comma should go before "which"--and that, when they use "which," "which," and more "which" in every other sentence, sometimes (when the phrase is necessary to the meaning of the sentence) it should actually be changed it to the word "that" and NOT use a comma to set off such a necessary phrase.

Anyway, I'm rambling.

What I wanted to say is, another thing I was taught was that, when in doubt, less is more and the aim should be for a clear and easily readable sentence. Technically, you could put ALL the commas in:

"Dr. Hunkyman placed the bronchoscope and noticed an endobronchial lesion, which, on fresh frozen section, looked like small cell cancer."

But I think it's clearer and easier to read if you just set off the whole phrase "...which on fresh frozen section looked like small cell cancer," with a single comma.

Always place a comma prior to the word "which" to set it apart. Does it stand apart as a complete thought? "which on frozen section blah blah." Yes.

I was always instructed this way.

This sentence would NOT have a comma after WHICH because the sentence stops and no longer describes anything.

IF.

Dr. Hunkyman placed the bronchoscope and noticed an endobronchial lesion which, on fresh frozen section, looked like small cell cancer.

YES this would be correct because we pause to describe the section and continue on.

This is how my first mentor described it to me instead of using "verb before adjective, after none" or whatever that totally confused me.
