Commas before "which" - FastMT

Does anyone have an easy way to tell if we should put a comma before the word which? The grammar website seems to complicate matters for me.

He doesn't know which way to go. I'm not sure which car is the best buy. The meeting is on a Tue. in March, but I'm not sure which Tue.
The pharmacy, which is across the street, is closed on Sundays. My car, which is parked outside, needs a new battery. We will celebrate on your birthday, which falls on a Sunday this year.

If it's necessary to the definition of the specific noun, I omit the comma.
For instance: "The car which I drive to work is in the shop." (defining which of your family vehicles you're talking about...if you're lucky enough to have more than one!)
I would use a comma if it was more like this: "My car, which I really need for work, is in the shop."
In a medical setting, the sentences might be something like: "The medication which he is using to treat his rash isn't working" as opposed to, "The medication he is using to treat his rash, which was just a sample size, isn't working."
Dr. Hunkyman placed the bronchoscope and noticed an endobronchial lesion which, on fresh frozen section, looked like small cell cancer.
Or should it be: which on fresh frozen section looked like.....

I've been marked off both ways, darned if I do and darned if I don't. ...
I wasn't sure if I should be using commas here and make this one long sentence, or if it is correct as is...also, I wasn't sure if "forward flex" is correct.
PHYSICAL EXAMINATION
Physical exam revealed that he can forward (flex) about 60 degrees. He can abduct to maybe 70 degrees. He has virtually no rotation.
