He also has loose bowel movements and fever of 102.0. The child was seen back on June 4, 2010 by Dr. ____ and at that point in time, he was diagnosed with pharyngitis and *:26. He was treated with OmniCef. The chest x-ray did show some bilateral non-specific accentuated markings without any space infiltrates and also bilateral maxillary sinus infection.
possibly otitis media? - sm

ear infection is a common dx when OmniCef is used.

She says otitis media frequently so I know what that sounds like with her accent. This is a new word for her to use. Thanks though.

There's a list of 8 paragraphs that begin with "Doctor diagnosed... " and then a list of conditions that the patient had/has follows each. Would Doctor-diagnosed be hypenated in this case? Thanks! ...
This dr. is not consistent.
Sometmes she says, "She is married with a male and a female CHILD."
Other times, it's "She is married with a male and a female CHILDREN."
This occurs often enough that I'm totally confused on the proper term. ...

I had to TYPE them, OMG! I type for a pediatric clinic and these name spellings are outrageous! The way my platform works, I type in the patient's name the way it sounds and after the dictation is done a search list comes up and I am ALWAYS having to go back and change the spelling I gave. I used to think it was all cute to spell a child's name different, but now if another, I may name her "Mary" LOL. I just typed on a kid named Jacob, but of course it was spell ...
Under "Review of Past Medical History" he says "Doctor diagnosed, etc." for several paragraphs. Is this a heading or does it need a colon after "diagnosed?" ...
One child has thyroid disease and "queshee acor." (sounds like!!!) Tried searching diseases and still couldn't find... which is hard anyway since I don't know how to spell it... Sure sounded funny!!! ...
