While patient was in the hospital, s/l *I-DAM-tee-oh-statin* level was ordered and was undetectable, and s/l *I-DAM-tee-oh-statin* inhibitor level was ordered and the level was 3.6 and elevated.

A statin is a HMC-CoA reductase inhibitor. So what would be a statin inhibitor? I'm so confused.

But he's definitely saying some kind of statin and then that same statin inhibitor.

Patient was initiated on rituximab so not sure if these results are before or after treatment, good or bad. ?

Dictates this right between sentence where rituximab was initiated and part where patient got better and was discharged. So don't know if they happened in that order. ???

You adding the part about rituximab was the last piece to the puzzle. Didn't search further, but statin could still make sense on the end, I think.

In all my years as a medical transcriptionist, I have never seen levels as low as these.  I went back and listened several times and I still can't believe what I'm hearing.  Have any of you every heard of levels this low: Cholesterol was 90, triglycerides 103.  HDL 6, LDL of 64. ...

I've seen something mentioned on here about putting DEP after your username to check how much work there is in the system, and something about someone sending out reports of how much work there is... probably the CCM I guess. I'm newish and I have NEVER seen a report of how much work there is and the DEP thing doesn't work when I try it (it says contact my supervisor for permission). Patient has HIV and is status post acute kidney injury: He has had no evidence for cryoglobulinemia or hypocomplementemia.  It will be interesting to see what his protein has done with improvement in his HIV parameters.  As per previous notes, I have not biopsied him.  Therefore, I do not know whether he has a high s/l VAN, high s/l VIC, or whether this is diabetic related kidney disease.  However, it would not change his management at this time.   ...

I just QAd an ILP report that I was only supposed to fill in the blanks, but I saw a major error near the blank, so I did a 100% review.  I found 32 errors, 17 of them were critical with missing phrases, words, and just plain making up what they thought they heard.  I checked the travel details of the report and it went through 2 levels of ILP QA before it got to me!  Why are we losing our jobs to them?  Oh, yeah, the suits make more money.  Never mind the patient with " ...
