I'm doing an operative report. The surgical assistant starts the report and gives the demographic information and then puts the phone down. Then, the surgeon comes over and starts dictating the surgery.....WHILE THEY ARE DOING A PROCEDURE ON ANOTHER PATIENT!

I didn't believe it yesterday when I had a report for this same surgeon. Today, I have this person again, and I have heard twice now the surgeon giving directions to the surgical assistant re: positioning of body parts, etc.

Am I strange for being just a bit disturbed by this?

A lot of the time the surgeon is just directing traffic, and the residents do all the work since they are learning. I have an orthopedic surgeon who dictates his op notes while the residents are still working on the patient as well. Weird, but true.

That's nuts! No, you aren't strange for being disturbed by this. That's like being on a 747 in coach and the pilot walks by having a cup of coffee while giving instructions to the copilot through the mic. It doesn't inspire confidence.

If the assistant surgeon is ready to do the surgery on his own, why does he need instructions? I'm picturing Dr. Nick from the Simpsons when he's doing Homer's heart bypass and he's singing "The kneebone's connected to the... something. The something's connected to the... red thing. The red thing's connected to my wrist watch... Uh oh."

I've seen a lot of incompetence (pretty much on a daily basis), but that takes the cake. Thanks for sharing!

When I go to my neurosurgeon for my back, I want HIM doing my surgery..I picked him for his education and background. I believe legally they have to ask your permission to have a resident do the surgery..Definitely a question I will ask from here on out though..I know they have to learn some how and I dont have a problem with it, but I would like the doc at the very least hovering over them, not on the phone dictating a report..

I want to think this guy is somehow physically participating in the surgeon. He is in the orthopedic area and says "give me a little more external rotation" and "give me some bend in that knee" in the middle of a report.

What's worse is that the impression I get from previous reports is that this guy is sought after as he has had patients from several other states come specifically to him.

The drama never ends in this field!

I think lots of things happen in the operating room that are flat out terrifying and wrong. I typed a little kid getting T&A and tubes. The surgeon put down the time of surgery as 4 1/2 minutes. Think how much the patient is billed for. Maybe that is an apple and an orange, but truly I think too many doctors think of their patients as comodities and not people.

After all of these many, many, many, many (okay, you get the point) years of transcribing, my wrists don't hurt, my back doesn't hurt and my neck is still okay, but the one thing that has me really bothered is my heel !!! I just heard that the 2 largest MTSOs in the world are approaching virtually every large to medium-sized hospital and medical center in the country with an offer for 10 cpl for transcription. I'm not even sure who the 2 largest are, but I'm pretty sure one of them is CBay and both are offshore companies to be able to offer that rate.
