I DREAD Monday's.  Coming back to see yet mandatory OT.  I am so tired.  It's all I can do to sit here 8 hours a day and now 9 or more.  I try to be positive but when I see the jobs I get, forget it. 

Same &*%#, different day. 

Wish we could get the doctors to care.  It would be great if  the sound quality would improve.  Static on line, turning away from mike, etc.  I am losing my hearing fast.  Turn the volume all the way, barely audible.  Next report, blow out your ear drums.  Wish the IT people would have to put up with this. 

I am so frustrated because there is absolutely NOTHING I can do to improve the situation.   It seems like every day is no better than the last. 

I envy those who are getting 250 lines plus.  With my accounts, such is not the case for me.  I agree with previous posters when they say tier 3 MTs get crap accounts.  Wondering if it is possible to drop down to a 2 and how much pay would be cut.

Looking for a fairy godmother to help me.  HA!

Me too. When I've been off for a while, I try to adjust my attitude, say it's the best thing for now, just hang in there. That is...UNTIL I log on for the next shift. I've miserable from the 1st job. Each second is agony. I, too, want to go back to Tier 2. There must not be many tier 3's anymore. I just get more and more op notes every day, which are the worst, fastest. So many accounts. Ugh. Started as Tier 1, now 3, and nothing to show for it but harder work, lower pay.

Typing ER reports all day long..I can do them in my sleep and at least they aren't the same thing day in and day out.

There's no tier training in MT school, right? We all learned by doing, why should anyone be exempt from learning how to do a specific worktype? I think it's more a matter of what someone is willing to do, not what they are capable of. As far as I'm concerned it's a form of cherry-picking.

I would take a survey and match people up with what they like to do, happy employees are productive employees after all..I hate OR reports, but as a tier 3, that is pretty much all I get. Wish I could drop back

an acute care position as a newbie, I learned the hard way as I went. It was a stressful couple of years & it took a while to get my speed up & not have to look up everything, but I learned it all as I went & did everything everybody else was doing. This new way is simply not fair.

I don't think the misfortune of getting crap accounts has anything to do with what tier a person is. I'm pretty sure it is just the luck of the draw. I thank my lucky stars everyday for my accounts and if ever forced to change will quit in a heartbeat.

When I first started at MModal several hundred years ago, I understood the whole tier thing to be as follows:

Tier 1 was for new graduates and less experienced MTs OR those who chose to work at this level. As an MT gained experience, they could "level up" to become a Tier 2 and Tier 3. Tier 3 MTs were given the most difficult reports (OR reports with the 10,000 different kinds of trocars, etc.) and would receive the highest compensation.

Someone earlier called the tier system a form of cherry picking. I do wish that was the case and someone would show me how to do the cherry picking because some of my accounts are just downright horrendous and my line count continually suffers for it.

You said yourself people could choose to stay at tier one. I used to be able to do 4000 lines an hour straight typing on an account with nothing but ER reports. The pay raise to higher tiers would have to be huge to justify making that kind of money on more difficult reports. I wouldn't blame anyone for staying at tier one and doing nothing but easy work. I just don't think that should be an option, I think once one is past the probationary period they should start becoming exposed to more difficult dictators and worktypes and the jobs evenly distributed to all so that everyone gets some easy and some more difficult work. There shouldn't be a reason for anyone to stay at tier one other than they can't make as much money by advancing to higher tiers, that is what I meant by cherry-picking. Anyone who can't advance beyond tier one shouldn't be a transcriptionist.

Straight typing, they do say the same thing over and over and over, just make a generic report and change it around to fit the patient..I miss my ER reports.

I haven't typed an ER report in more than 10 years. Now I'm lucky, very lucky, if my line count is half of that. I wasn't bragging by any stretch, just trying to make the point that if you get all easy reports in tier 1 you can do really well, why would anyone advance if they didn't have to? More difficult time-consuming reports at even $0.02 per line more still equals less pay, especially now when most are on many accounts. Back in those days everyone worked on one account, at least in our office. (This was before anyone worked at home; reports were printed and copied in our office and hand-delivered to the hospital or clinic. Different universe, right?)

first for a company working off Word 5.0, I figured out how to make generic reports and from then on it was game on. Started here doing ER, left and came back, wish I had tested down last go round.

not my line rate 25 years ago. I don't think there should be tiers. If it's true that there are sound issues and multiple account specs at tier 1, then why should they get paid less than tier 2 or 3. And why shouldn't everyone learn all worktypes? I agree that it would be nice if we could all just do only what we wanted but let's face it, there's a lot of crap out there nobody in their right mind would volunteer for. So the point I was trying to make was why shouldn't everyone get some good and some bad, instead of some getting all the crap and some getting all easy work? That's all I'm trying to say, albeit apparently not very clearly :)

Some get cream, other's get curdled milk. Me, I get curdleed milk. I don't even bother sending e-mails to HDSM as nothing changes. Too bad we can't all get some easy accounts once in a while.

I am so tired of struggling to hear and wasting time tryng not to leave blanks. I can remember a time when I used to have a fairly easy primary but that memory is fading. FADING FAST!!!!

Work is not divided out equally and never will be. That's corporate for you. I keep hoping this will change some day but know that in all likelihood it will just keep getting lower and lower.

If the dictator is bad, it doesn't matter what type of report.

I thought the difference between tier 2 and 3 was op notes only. Shouldn't everyone get the next report in the queue unless it is not in your tier level? I don't see why dictators would be divided up. That doesn't seem right.

I get whatever job in my account is next, but am a level 3 so I get every kind of job in my account that is next in line. I get bad along with good, but fortunately my account, on the whole, is pretty good.

You don't see why dictators would be divided up? How is it fair for some of us to get the worst dictators 90 to 100% of the time.

I bet this scenario has happened to more than just me. You are on an acounts or accounts and as soon as you get it to a decent level and understand the nuances of the more difficult dictators, they (don't know exactly who has I have had so many supervisors in the last 6 months) that I have lost track, you get put in anothe POD with absolutely lousy dictation.

The quality of the dictation does matter, and poor dictation brings line counts down I just wish more people would admit this, and stop talking about short-cuts and such.

I meant that I don't think certain dictators should be assigned to certain people. We should all share the dictators. That's my opinion. The tiers should only have to do with worktypes, not with dictators. I thought someone said that tier 3 gets the most difficult dictators.

I agree that production has to do with the quality of the dictation for the most part.

this is great news for Mt Laurel New Jersey.  Are there any other offices who have done this?  they actually qualified and that means they have set a precedence.  Everyone who is struggling with Medical Transcription right now either losing their jobs or (sounds like) had their hours cut should check into this... I just found out about this government program.  From what I understand, it helps those who have lost all or part of their hours/income due to outsourcing of their profession, especially us older workers who need to retrain in order to reenter the workforce in another field.   It looks to me like Medical Transcriptionists should fit their description, and we should be eligible for some funding to get some training!   The catch is, in order to file, you have to have at least 3 pe ...

On clocking out today, my min wage adjustment went up.  How is it possible that they know what it should be for the entire pay period 2/1 to 2/15.  I'm sure I've made more than MW.  This is just baffling.   ...

I used to be a person with such a positive attitude, always smiling, taking the good with the bad and never letting the bad get me down.  Since all the recent changes at Mmodal going back several years, I've become more negative, more cynical, more frustrated, and more sarcastic.  I no longer care about the quality of a report when I get a dictator that obviously no longer cares about his/her patient.  You know that dictator that has the nerve to call on his/her cell phone wh ...

Two days in a row now of the worst ever ESL dictators.  Sometimes this jobs just makes me want to SCREAM loudly but that won't solve the problem (s).  Too many days I have sat her crying over the quality (or lack thereof) of the physicians dictating.  They just don't care how they dictate.  WE are responsible for making them 100%.  So very tired.  Fondly remember the "good ole days" when I first started with Your Office Genie.  Each subsequent buyout ...

My very own attitude has gone from bad to worse with the 4 cpl and the fact that I am typing over most of the garbage.  I was told by my CCM no F11 unless technical problems and they must be reported. Again today, I find myself really very angry while I wait for a job outside of the house to come through.  I find myself angry that I sit here for 4 cpl and no matter what I won't be able to make bills, b/c I am again straight typing half of this stuff and my skill set/self-esteem k ...

According to a poster below which has apparently already attended one of the payroll meetings (as the poster posted) for nuance they have decided to keep the incentive part of all of our pay going with an exception that now you have to have over the industry standard of 98% to get a production based incentive. In addition (if this poster is accurate) they have again cut the overall pay from 8 and 4 to 3 3/4 AND 7 1/2. This is nothing but a bunch of crapploa...an MT who is now getting ...
