Where can I find the HIPAA rule/violation of when a doctor dictates the patient name in a report.
Its not really a HIPAA violation. it is a matter of confidentiality. sm - no name

some hospitals allow the name in the reports where others do not. Only if someone got their hands on the report itself would there be a HIPAA violation. Just having their name in the report would not constitute a violation. Also if you feel uncomfortable with putting the name in, just substitute it for "the patient". That is the common practice.

to actually use the patient's name in the report about the patient whose name is used in the report? What purpose beneficial to the patient could possibly be served by not connecting the patient to the information about him/her?
You may have worked at a facility where it was their policy not to put the patient's name in the body of the report, but that was just policy. It had nothing to do with HIPAA.

Every major medical center I've worked for in the last 30 years have wanted the patient's name in the report if dictated. It would be a big no-no to substitute "the patient."

HIPAA violations involve *disclosure* of PHI/PII to inappropriate parties. Unless something has been disclosed, no violation can have occurred.
Physicians can include names, numbers, and anything else in a report. There are limitations placed on that.
Thinking that omitting the patient's name is somehow going to save the facility from HIPAA violations is silly, because it won't. That is because identity can still be determined by combinations of other information.
FYI, typing cc's to Joe Schmoe is not in itself a violation. You just typed what was dictated. You cannot be responsible for knowing which Joe Schmoe was intended. The fault lies with the facility which is stupid enough to still be faxing copies all over the place.
What do you think of a company that tells you, as a rule, to put the patient's name in the report when stated because the hospital requested it despite HIPAA? I haven't been doing it, and they haven't called me on it yet. They probably didn't notice. ...

Can anyone state the rule for proper usage of a or an? I know you use "a" when the noun you are referring to begins with a consonant and use "an" when the noun you are referring to begins with a vowel. However, is there a rule that applies to usage of a or an when it pertains to the below?

The company I work for is not real strict on QA but they do check it monthly. I had usually gotten 99%, but the last time it was 97%. Next sentence has same word: "EKG on arrival to the emergency room: Sinus rhythm with no acute ST-T ___". This time is s/l it begins with an "R". ugh!
