Currently, I have a FT job w/a very large hospital, actually multiple hospitals combined. We all once worked in the office; however, they sent us all home to work. There are about 130 hospital transcriptionists. We also outsource a lot of work to a large company. There have been so, so many changes - everything from new management, VR, pay for production.....Communication amongst co-workers has disappeared. Management doesn't respond. I'm really wondering if the hospital is just trying to transition to entirely outsourced. Why would they keep in-house and outsource? Do a lot of large hospitals do this? Does the outsourcing company want to hire those transcriptionists? Any thoughts on this appreciated.

This happened at our hospital. Eventually the company that handles the overflow from what in-house or hospital employee transcribing from home will probably take over entire transcription (or a company biding lower) and yes they keep employees, at least the one that did our hospital did (Transcend). However, pay went down dramatically and no longer hospital benefits. For example if I had to go to the hospital when I was a hospital employee that was all free. That benefit no longer exists!!! I am afraid it is all going out to services. I too work for a large hospital group with many transcriptionists all at home now and your story sounds just like mine. I too started out in house and then went home and eventually they ended up sending everyone home. We had a large backlog that was being sent out to a service as well. Our upper management saw how much in house plus outsourcing was costing and started asking a lot of questions about why pay in house people, pay benefits, etc., if they can outsource for cheaper. I have also had the pay cuts with introduction of VR and the whole bit. They have looked into outsourcing previously but it was back a long time ago before overseas workers drove down the cost so they always kept us because costs were similar and they liked having control of the work.

Unfortunately, the outsourcing company hiring back all of the in house transcriptionists tends to work in management's favor when deciding about outsourcing. They will maintain their workforce, quality of work and not have to pay them anything, especially benefits. They just pay the outsourcing company one fee and not worry about it.

I had been making a very good living at this. I have looked up most of the companies I think may get our work if we do outsource (we have been told we currently have our jobs until the end of the year, then they'll let us know) and they do not pay an amount that makes it worth it for me to stay with them. I don't see even IC positions paying what I am used to :(

Well, I have a slim hope, but the word we got from middle management is that upper management was leaning heavily toward outsourcing. My guess is that the contract with the networking service we use will expire at the end of the year, which is probably the main reason for keeping us until then. Management has cut our salary to the bare bone with VBC pay/no spaces/no headers. I don't know how we match up with the services' undercutting / underbidding tactics, but with benefits, I fear we are still coming in higher. I have been with this hospital many, many years and we have never been this close to being outsourced. It is really depressing to think I may have to start all over somewhere else. I do know though that I will absolutely refuse any offer that is below what my experience level should bring or any schedule that includes a shift EVERY Sat. or Sun., which means I am probably out of business as a transcriptionist. I don't know what I will do, but I refuse to slave myself out to a service where they barely pay a respectable salary and where I am treated as just another number. I have always treated coworkers and management with respect and I demand that I am treated the same way. With my years of knowledge, I refuse to be paid at the same rate as a newbie or someone who can't do as many work types, etc. If they cannot/will not comply, I will walk away from it and just go from there, wherever it leads.

Because all that you mentioned that you would refuse - is (sadly) all there is, that or nothing. many of us have refused as well, to no avail.

Just thinking out loud here. I know many practices probably do their own outsourcing to MTs at home. Why not more? It must be cheaper for them with higher quality work, personalized service, etc. Some hospitals even hire MTs to work from home rather than use an MTSO. How can we, as the MT community, reach more clients/practices. I'n new to working from home, only 3 months now, but have been a transcriptionist for 6 years. The company I work for has me on 5 different hospital accounts. They are all rehabilitation hospitals (acute) and do H&P, discharge summaries, daily notes, consultations, etc and I am constantly out of work. Some days I have no work at all.
