Does anyone know anything about SOAP transcription, or QED? Thinking about applying. Thanks

subjective (history) objective (physical exam) assessment (diagnoses) and plan (recommendations). It is usually abbreviated, can be single or double spaced between categories, depending on the particular client's preference. Some will say "impression" instead of assessment, but expect you to use the A: for the heading. Here is an example of a SOAP note:

S: This 43-year-old lady comes into the office today for complaint of migraine headache. Please see previous notes for the rest of history.

O: Her pupils are equal and reactive to light and accommodation. Nose is clear. Ears are unremarkable. Mucous membranes are moist. The rest of the exam is unremarkable.

A: 1) Migraine headaches. 2) Hypertension. 3) Hypercholesterolemia.

P: Gave the patient a prescription for Migranal. I have recommended she get plenty of bedrest in a quiet, darkened room for the rest of today and tomorrow. Call for worsening headache, nausea, vomiting or other complaints. Otherwise follow up in 1 week.

Again, there are many preferences within the SOAP format, but this is the most common style. I have done SOAP accounts, however, where it's the same as above, only they want the words typed out in all caps (SUBJECTIVE: OBJECTIVE: ASSESSMENT: PLAN:) rather than S: O: A: P:

If you don't mind being micromanaged and interrupted every few minutes while you're trying to type, and your every keystroke watched and potty breaks timed, then working there shouldn't be a problem.

Never worked there, but they invite newbies to work - on fast-turnaround reports, so

some micromanaging might be appropriate and even helpful for the right people. For people with enough experience, I couldn't recommend any position that didn't provide extensive broad-ranging dictation types. Those little notes some like to do for physician practices are soon going the way of the dinosaur.

I have over 15 years of experience in this field. Tested with SOAP just to be told I am not a "good fit". Is anyone else out there tired of taking the ridiculous tests and questionaires? I'm curious about something and I hope you all can clarify it for me. I've always understood that when using a SOAP format, you do not use headings such as HPI, physical examination, review of systems, etc. I've got a situation at work where initially we were to use HPI, ROS, PE, etc., no matter what the dictator said.
