I finally found it! Yeah for me! LOL. It is SOAPP-R test. I had to go back to previous reports in January to find it. Thank you very much for your help though.
Lol, good for you, had a phone call in the middle!!.....nm - Cyndiee
Dictated: Lumbar transforaminal epidural injection. Use "Magnahist."
He says it clearly enough. On search, I get a few sites for "MagneHis" but not enough information to confirm.
Any help appreciated, thank you. ...
Just curious if you were able to confirm that it is a legitmate ad since the company's web site states different and the job ad remains on the board.
Thanks so much!!! ...
just want to know what's up with that job post on the job board. I've sent my resume twice and came back as undeliverable. I got so excited because I have experience with PMs. Now I'm disappointed. Any thoughts?- tx, nw ...
Are there any billers/coders out there who specialize in pain magement, ie, facet injections/aspiration and lavage, etc, for outpatient pain management physicians?
I have questions about documentation needed for injections and such with the 'add on codes', for example with the injections, there are codes for the first level then additional codes for second and subsequent levels. Is it necessary for every single charge to have a report on it? It's causing severe headaches for the ...
He has positive s/l marie fwa reflexes and there are a few beats of clonus....
He says it so fast - the "marie" also sounds like "murray" and the "fwa" is like the foie in foie gras.
Any help? Thanks! ...
The latest HCPCS code update that went into effect on July 1, 2014 is important for pain management practitioners and coders since it modifies the way you report some morphine and propofol administrations. Read more.. ...
I'm going completely blank on this one. The patient entered the hospital for chest pain. The doc states: (blank sounds like gel, or jail)
He denies any current discomfort but states when it is present it is usually a mild to moderate degree of pressure about the chest. He denies any radiation to the _____, arms, or back. ...
Started in 1973 and my passion has remained steady over the years, a few years not so much. Seeing the change and didn't like it but it was something I couldn't walk away from. Set out to become as proficient as possible; do every worktype, every specialty. I wanted to be good at all of it..to be wanted, needed, and sought after. Also offered the security I needed when I was younger, single mom;l life not so secure. Being an MT was my haven, my source of being grounded.&nbs ...
When you are using a SOAP Chart style, do you every use PHSYICAL EXAMINATION in the headings? I have a doctor that added it in today with his dictations. I have never used it before in a SOAP. Thanks ...

I have over 15 years of experience in this field. Tested with SOAP just to be told I am not a "good fit". Is anyone else out there tired of taking the ridiculous tests and questionaires? And when you are a "fit" somewhere its at 6 cpl. Seriously? They want all our expertise, we pay for equipment, and now have to be IT specialists. Does anyone know of a decent company out there anymore? ...
I was just looking at the careers on Nuance's website and they have a position for "Soap Absorption MTs." Can anyone tell me what that is? I've never heard of it and have been transcribing for a long time. ...
Forewarning and forearming on these two companies. SOAP subcontracts to Care Tech -- HEAVY, heavy ESLs and I have 30+ years experience -- I got hired three weeks ago, started last week and the transcription manager of Care Tech, was "on and off" with school, days off so on and so forth. Thursday, I got the first NASTY virus of my "computer days" -- Ezula - it embeds on a toolbar -- I think coupon stuff. Anyway, I tried to "clock in" -- ...
Just curious because I did see the ad that SOAP had posted and maybe I missed it, but I did not see anything in the ad about them supplying the computer. I had a similar situation with using my personal computer to look stuff up on and the company computer for company stuff, but I have to agree, that it was a bit harsh on their part. I was just wondering why you were supplied a computer, if there was no need to use one of theirs. ...
Has anyone else out there applied for one of there many positions and not heard anything? I applied for a few of them over a month ago and have not heard anything. I even sent them notes wondering if they got my resume and have not heard anything. ...
I hope this is correct board to use.
Okay, I have done both but I want to know officially which one is correct. In a SOAP note if the doctor dictates "Clinically the patient has a herniated disc" does this go under objective or assessment. Thanks for your help on this.

Hello, can anyone tell me of a good company that just does SOAP formats for clinics? Trying to find something to work on evenings and weekends. thank you ...
I have a interview Friday with SOAP Tanscription and wondered if anyone can tell me a little about what they look for/ ask in the interview. Thanks so much! ...
I'm curious about something and I hope you all can clarify it for me. I've always understood that when using a SOAP format, you do not use headings such as HPI, physical examination, review of systems, etc.
I've got a situation at work where initially we were to use HPI, ROS, PE, etc., no matter what the dictator said. This was for a variety of reasons but a lot of it had to do with the coders. Now, the directive is to use the heading the dictator says.& ...
