I just started with a new company and am really depressed about the "corrections" I am getting back from their QA. I have 35+ years of experience and yet I can't seem to do anything to please them. I'm told to type verbatim, but when I do, they "correct" that, stating I left "the" off when it was never dictated, etc., and other stupid things similar to that. I am to the point where I wonder if I will ever please them because it seems that no matter what I do, they are not happy. I am very frustrated and depressed and I needed to vent.

We must work for the same place. Their idea of verbatim and my are totally different. I think you have to be a mind reader to please QA.

I even corrected grammar in a sentence and was "corrected" to put the wrong thing back in. Since most schools don't even teach students proper English any more, I think some of these young QA girls don't have a clue what proper English even is!

I finally just stopped paying attention to "QA" - They are all different all the time.

Unless there is some gross error I just do the best I can do, and if they don't like it, fire me and give it to the Indians or VR and see what you get. Definitely not worth getting depressed over any more.

And spoke directly with the top person, above all QA. She said to me that each QA has their own "style" and I only had to worry about the important corrections ... correct MR #, correct lab values, and things of that nature. The rest of it was not something to worry about and it would not lower by QA rating.

I get "major" and "critical" errors for typing verbatim and not correcting things like incomplete sentences. These scores should be given for things such as incorrect lab values, drug errors etc.

I've been doing this 25 years and never had a problem with almost immediate release from QA...then I went to work for one of the larger nationals.

At first the QA person in charge of the account to which I was assigned sent me instructions saying everything was verbatim with "gentle" cleanup. No problem. Or, so I thought.

The account was being trained from straight transcription to eScription (a program with which I was very familiar, which is one reason I was hired). Well, lo and behold, my first day I get these QA notes back from her clearly being verbatim ONLY and not allowing for even the most gentle of clean-up.

When I questioned one particularly glaring error she had reinserted into a report, her response was (and I kid you not):

"Well, we have to transcribe it EXACTLY as he dictated it, after all we're training the system to recognize voice and that's of utmost importance."

Ummmm...more important than sending through a CORRECT document? This "seasoned" QA person actually was instructing MTs to send work through knowingly riddled with dictation errors because the system was being trained. Meds, names, incorrect terminology...it didn't matter, she wanted it transcribed 100% verbatim, no blanks, no corrections.

Craziest thing I'd ever heard...Anyway, don't feel alone out there, OATMT. Sometimes we get our hands slapped even when we're doing the right thing!

It means verbatim unless another rule supercedes verbatim typing. Our client profile has many preferences of the client, so we don't necessarily do verbatim 100% of the time.

When training a sysvem for VR, you do have to do verbatim so the system learns syllables appropriately. People who don't take the time to train their VR system right pay the price in the end. And may God help you if a PA or ANP uses their supervising doctor's number to dictate on VR. That really screws up the voice file for that doctor.

Training the system NEVER supercedes accuracy of the report. That thinking in and of itself is dangerous.

This is where "back-end training" of the system comes into play.

To suggest knowingly producing a document riddled with errors just to accurately train the system is foolhardy.

I, too, have worked with eScription a great deal. Reports are to be transcribed verbatim yet ACCURATELY, with either blanks left for obvious errors or the correct word transcribed and cleaned up on the backend.

Amazing how quickly some MTs are to throw accuracy to the wind. No wonder patient care is in the gutter and we are being replaced by machines.

I empathise with your frustration. I do not have as many years of experience as you, but I was a new hire eight months ago on a verbatim account. It is not strict per se since they allow tweakage to help it make sense, and we have a lot of ESLs, but I felt pretty discouraged during the first few weeks with all of the "red" on my reports. They were for the same silly things, such as "exacerbates pain" vs "exacerbates the pain."
One day I realized the only way out of the frustration was to ask them to define their expectations. I e-mailed my TL, who brought QA on-board, and we discussed specifics in a friendly and productive way. Problem solved!
It is easy to spiral downward when you're working alone and the QA hits just keep on coming. Try to be proactive and take the next step out of the gloom.

She only wants to change my schedule which, according to her emails, would be foolish since the times she wants me to move to have no work either. I would go above her but what good would that do? I find that lately I am feeling depressed over the reports I've been transcribing. Maybe it's because it's the holiday season and I can't help but feel badly for these people who are so very sick (I do acute care). Lots of oncology these days, for some reason. I have never felt more worthless in my life. I work for about half the pay, it takes longer to do because the drafts are so awful. You get marked down if you don't use shortcut keys to navigate or whatever and the object is to keep you fingers on the keyboard, but you cannot keep them in the position you need to for transcribing or fixing all the problems.
