Dear doctor (sarcasm) - need to vent

Thank you for dictating your three H&Ps starting at 1 a.m.  on patients who were admitted yesterday and the day before (so therefore are already out of compliance on).  I appreciate the yawns and am really grateful that you apologized for coughing in my ears.  Would really have preferred that you take a nap first, then I might actually be able to understand you wouldn't have to leave so many blanks since you aren't that good as a dictator in the first place.  Maybe next time.  Sincerely, your underpaid, annoyed and cranky MT

I do editing and there is one dictator who is 10 times worse than all the others. He too dictates during the middle of the night and snorts, burps, etc. When I see his name on my queue, my heart sinks. I am very luck to have many samples and I work very hard to transform his report into a masterpiece. I must admit I have felt unkindly towards him on a personal level, feeling that his lack of consideration for the MT/editor he has never meant was purposeful. Recently, I was editing a report from the same doc. The patient was refusing a surgical procedure that he needed to save his life. By comparison, this dictation was very clear and I could hear the compassion in the doc's voice. I still don't like editing his reports, but I no longer take it personally. I began to regard this dictator as a human being--a very nice human being, a human being I would like to be my doctor.

I do think doctors used to dictate better when the MTs worked in the hospitals. When they knew you could track them down easily in the doctor's lounge (that was right next to medical records), they would behave themselves. Also, as I recall, doctors used to not be burdened by so much paperwork, so they weren't up at all hours doing it. I think I heard somewhere that docs spend over 3 hours a day just doing documentation. No wonder they mumble sometimes.

I know it doesn't do any good to talk about the good ol' days, but I do remember having access to every dictator and 100% of the time I made a request for them to slow down or to explain what they were saying, they did gladly. I think the lack of communication between MTs and dictators is unfortunate. Coders need only shoot a physician an e-mail to clarify a diagnosis. MTs no longer have that luxury.

We have a doctor on our account who is a challenge at the best of times... but he dictates a lot late at night, it's quite obvious he's dictating in his sleep, he's practically incoherent, and has been known to dictate a medication list or a physical examination 2 or 3 times (and they contradict each other).

On a team phone call, one of my co-workers declared, "If he's going to dictate in his sleep, then I reserve the right to transcribe him in my sleep."

And I've always said, if they don't seem to mind their reports coming back looking like Swiss cheese, so be it!

Rambles on, makes no sense throughout the dictation at all, does not make complete sentences, dictates on a patient in the WRONG chart constantly, follows no format, which is required by YOU, cuts himself off but yet when you can understand him, he has a "bright" moment and makes sense is an absolute joy to transcribe????  Oh, and did I mention that he only dictates every third day even though he sees patients on a daily basis???? Mini-vent over, back to the confusion ... TIA ...

I got in trouble today because I put a wrong patient.  The doctor dictated a billing number, name, and MRN.  The MRN was had a different patient, but the billing number and the patient name matched the ADT screen.  Not to mention the fact that he kept using "she" in the dictation and the MRN number matched a "he".  So logically one would just think he had the wrong MRN and I didn't want another QA submission so I sent it through with the correct billing number, name, and ...

Can anyone tell me what the policy is for taking time off for a medical appointment.  I have worked here for over 5 years but never done it.  I am a regular part timer who just switched over from flexible part time so I have no PTO accrued to use for the hours I need off and really just want to take it w/o pay.  Just curious what others on this forum have done... ...

I am a former Spheris MT, now begrudgingly a MQ MT (not that Spheris was all that great either).  As was pointed out below, we are still on Spheris rules for the most part but I don't expect that to last forever.  At Spheris, if we think a doctor has made a mistake, we are encouraged to flag it but that entails putting in a blank, just as if we didn't understand the dictator.  If we have too many blanks in a given period, we lose our incentive pay.  They do not diff ...

Do you feel compelled to talk back out loud?  I have a new doctor that talks to me, and if he says thank you, I find myself saying "you're welcome" out loud!  He also asks questions, as in, "did I remember the vitals" and I say "no you didn't".  My dog kind of looks at me funny, like, are you talking to me?  ...

Okay, we've got this new French doctor at one of my hosptials. When I transcribe his dictation, in my head, I'm transcribing dictation form Gelles Marini from Brothers and Sisters and Dancing with the Stars. He's hard to understand, but I'm in love with him. LOL. It's a nice break from the other ESL doctors from the Middle East. Doctor Who Uses Tapes

I have a doctor that still uses tapes and refuses to go digital or call-in.  The problem is there is interference on the tapes and I can barely hear him.  They think it is my transcriber, which it could be, but I don't think it would be worth the investment to buy a new transcriber for tapes when hardly any doctors even use them anymore!  I need some opinions from my fellow MTs on this subject!  I risk losing the entire account of 3 doctors (2 are digital) if I don' ...

The AMA site, you have to type in certain code words before you get in...the UC Health site,  you can't just type in a name or part of name, you have to scroll through a long list.  Anyone got a way to find doctors in Florida that is simple without having to navigate through typing in codes for site access ...etc. ...
