unless there was a parenthetical phrase following the name as in "Steven Smith, MD, (we dropped the periods in MD years ago)an orthopedist, saw the patient on Friday. Commas around 'an orthopedist' as the parenthetical phrase. I think your QA is confused. I'm going to do a quick search, but I don't remember using a comma ever in that situation.
