This also applies to HPI. If the provider is talking about the patient's symptoms and listing them, only 6 shall be listed in one sentence. Then you start a new sentence, even if there is only one symptom left. Also as another PF MT stated, we are not allowed to combine positive and negative statements in HPI and ROS. This also goes along with many, many weird rules.

If the provider dictates no nausea, vomiting, constipation, but has diarrhea. You have to type: No nausea, vomiting, constipation. Has diarrhea.

If the provider dictates no fever, chills, malaise, sore throat, nausea, vomiting or diarrhea. You have to type the sentence as stated except for diarrhea because it is the 7th item. It would be in a new sentence ... No diarrhea.

This all goes along with pages of weird rules. You literally cannot type a single sentence at PF without asking yourself about the arrangement of words, if there is redundancy, if the word "some" is truly necessary, counting words ...

They may have started their list of rules years ago to aid in clarity, but is now beyond absurd and seems more of an effort to exercise complete control of some sort.

A lot of this stuff you do get used to, but not all of it. You can never just relax and type, at least in my opinion. The good news is a lot of the providers are very repetitious and you can make many normals.
