I think they are under stress and not sure I would be able to dictate that well myself. I have had some bad dictators who ate while dictating but I just thought maybe their schedule is so tight they need to eat on the run. Not pleasant but forgivable. Same deal with ESL dictators. They are way ahead of me knowing 2 languages and being a doctor, who am I to judge, however, it is still unpleasant to listen to them searching for words or using improper English.
