Yes, I have experienced that several times, and you shouldn't take it personally. No account gets 100% review over one person's error.
Most of the time, it's a client who complains (and nitpicks) constantly. They may want to leave M*Modal or are hoping for a price drop.
Audits are done on everything but you've had a lot of experience. I'd check the account specs for anything at all questionable, no matter how small, and send anything unclear to QA.
You'll get through this. Keep your head up and don't let them get to you . . . trouble like this is on the HSDM.
