I'm pretty sure now it's bowel sounds. These aren't regular long exam reports. They are brief, half page office notes, so no detail for body/systems. Just quick SOAP. I've kept typing it that way because that's the way I started doing it for him, thought it was right but then he seemed to change the way he dictated it. I'm going to try one more shot at asking him, too. Thanks again all of you!
