I have the Cigna TOS HSA plan for me and my girls, no spouse. It is 162 per check. Prescriptions are under the plan, but we never need prescriptions so I have no idea how that works.

There are in network and out of network rules. We also use the in network because it is cheaper of course.

Dental is not included but we have dental plan with MetLife because that plan allows you to use any doctor, and it is 16.14 per month. Four of us go to the dentist twice a year, and occasionally I will get a bill from the dentist for $20 for something that wasn't fully covered, but for the most part, I never have to pay any dental bills. Fortunately, we never have cavities, so it is pretty straightforward preventative care.
