I was marked off on audit for adding heading - sm

And I added paragraphs on an OP report, at incision and at closure as Ive done for my 22 year career, and was told unless the dictator says "paragraph," do not create one. Unless doc says "History of Present Illness," do not add it. If he says "medications include Tylenol and Synthroid," I am supposed to type it as follows:

MEDICATIONS:
Include Tylenol and Synthroid.

I was told even if the account is not verbatim, I better type it as verbatim.
