Inside the Create Entry screen, hit Ctl/K. At that "Next Key typed" message, hold down Alt and H together. You will then be in a screen where you can hit "Okay." That puts you back in your Create screen and you will see Alt/H in brackets. Do the same thing for each bracketed item.
