a parenthetical remark giving the doctor's specialty and would not capitalize it. If it was part of an address or sig line, I would take it as shorthand for "Department of..." or "Division of..." and would capitalize it. I'd request one of my MTs do it my way, but I certainly don't see it as a "dingable" error.
