There is no "the" in front of this, a case could just as easily be made that this does refer to the department as a name like an entity/person, so capitals would be fine.

Not to mention, it would look really weird to say Jim Smith, MD, infectious disease. Sounds like you are calling him an infectious disease, lol.

Lots of debate around this, though, so I don't think you would win if you challenged it. People will argue it strongly both ways.
