Sometimes the dictators dictate in handhelds that they later plug into the system and the handhelds retain the date dictated. Sometimes MM just gets overflow work that onsite can't handle. Sometimes work is just uploaded by the client late. There are a lot of reasons. I've gotten work that was dictated weeks or even months beforehand.
