I had one who frustrated me every time I got one of her reports. She would back up and run over everything she said, sometimes erasing whole paragraphs. One day, she made me so mad I yelled. After dictating 3 pages of words, she suddenly said "oops, wrong patient, please take all of this out. I have to try again." I almost quit that day.
