There are 3 facts here. The fact that he works at Lowe's is what's causing him to be on his feet all the time now, so I'd put those two together, and use the comma to separate the fact of patient's prior business and current employment.
Apparently wasnt on his feet all the time with his own company, I'm guessing, only now at Lowe's.
This is interesting, and I hope someone who really knows their stuff can share their opinion.
