I have an ESL doctor who's dictating dialysis bath solutions.  (I've never done these before).  He says 2 K, which I found no problem, but when I sent it to QA for some other blanks, it was corrected to 2 potassium.  Account info says to use abbreviations when dictated, even in the diagnosis/impression/assessment portions.  Is K the preferred transcription, or should I expand it?  Our QA is known to change correct transcription to incorrect.  Don't ...

Need Help For DQS Function Regarding ALL CAPS!

Good morning ladies and gentleman from MQ.  I need your help. On the ASR reports, how do you ALL CAPS a sentence? Here's the deal . . . Say under allergies, the MD dictates no known drug allergies.  What I do is hilite the sentence and then Control, Shift, A.  After I do that, the sentences comes out No Known Drug Allergies (initial caps only).  Then I have to do the Control, Shift, A again and it will switch to ALL CAPS. BOS Rule For Drug Names In All Caps?

Does anyone know the BOS rule for drug names that are technically all caps, i.e., TUMS, GRALISE?  Do they stay all caps or is it okay to capitalize just the first letter, or is it company/client preference?  QA is telling me just to capitalize the first letter so as not to draw more attention than needed to the drug name in the report, but I can't document this in the Book of Style.  Thanks in advance! ...

I know that "dialysis-dependent" requires the hyphen if a noun follows the phrase as in, "The dialysis-dependent patient..." However, would the hyphen be appropriate in this phrase if a noun did NOT follow it as in "This is an 81-year-old woman with diabetes who is dialysis-dependent."? Would this be considered a noun + adjective, thus requiring the hyphen? ...

A few weeks ago, for no reason that I can find, I cannot get the first letter of a sentence to be in all caps automatically, (unless I do it manually.)  The same for the letter " I " by itself when typed.   It was always automatic and now all of a sudden it is doing this and I cannot figure out how to change it back.
