Pediatric patient with earwax accumulation. ENT: Shows mild wax accumulation but both tympanic membranes appear healthy. IMPRESSION Wax (s/l inspusation) in the ear.

Thank you so much. I ran in to this earlier in the week and my QA opted to go with "accumulation."
I could not disagree, as I could not come up with anything else. ESL doc, but really dictates very clearly.

doc refers to mom or mother/dad/grandma, etc.  I was taught that if it is "the mom" you would not capitalize but if you can substitute a name, then it would be capitalized.  The mom denies vomiting, but Mom states the baby..... (would that be correct as far as capitalization goes?)    ...

This is a tough one. The patient is a child who is being intubated.  Doc says, "Using standard technique and following the dosing guidelines from the s/l Brassler tape, the patient was intubated." So I don't know if this is like a cassette tape that the doc played to reaffirm how to intubate a small child or if the "tape" is actually an instrument or what. Sorry if this is the stupidest question imaginable, but I have absolutely no clue. I swear to all that is holy, this is what this less-than-stellar PA just dictated:   "The patient IS a high-risk gestational trophoblastic disease..."  I went back and listened thinking I must have heard wrong and she said patient HAS ... Nope.  She said IS.  This is the same PA who dictated a creatinine of 92 for someone.   That sound you hear is me hitting my head on the desk. ...

Today out of the blue, got a call that I had done great on testing and they would like to offer me a position.  I said thanks but no thanks since I'm no longer looking for a job.
