Is there really a difference between 'neuroforamen' or 'neural foramen' or does it just depend on your client instructions?

if dictated that way, your doctor wants it that way. It is the same as tarsometatarsal versus metatarsotarsal, both words, one usage common, the other isn't, but both are proper usage of medical terminolgy basics. Just because it isn't in a "dictionary" doesn't make it not a word, as long as its proper usage of medical terminology basics.

"Neuro" cannot be correctly combined with foramen because the foramen itself is not a nervous structure.

"Neural" is used to characterize the foramen in this case.

because it relates to the heart and chest. You wouldn't say "cardiothorax", for example, as it doesn't make sense.

Similarly, you can say distal septum but not distoseptum.
